精品论文 参考文献 三维心脏超声定量分析冠心病介入治疗前后心肌运动及心功能中的应用价值研究 孙文兵　李秀兰　丁波　袁丽　熊俊　肖艳 　　湖北省江汉油田总医院超声影像科　湖北　潜江　４３３１２４ 通讯作者:李秀兰,主任医师,硕士研究生 　　【摘要】　目的　探讨实时三维超声心动图在冠状动脉粥样硬化性心脏病(冠心病)介入治疗术后随访中的应用价值.方法　选择我院冠心病行经皮冠状动脉介入术患者２８０例及未行PCI治疗的１２０例患者作为研究对象.采用三维超声心动图分析冠心病患者PCI前后左心室收缩功能和左心室收缩同步性指标变化.结果　PCI术组术前左室整体舒张末期容积(LVEDV)、收缩末期容积(LVESV)、左室射血分数(EF值)与对照组差异无统计学意义(P＞０．０５);PCI术后６ 个月LVESV 低于同期对照组的LVESV 值,差异有统计学意义(P＜０．０５);PCI术后６个月EF值高于同期对照组的EF值,差异有统计学意义(P＜０．０５).结论　PCI术有效改善冠心病患者的左心室功能及收缩同步性;三维超声心动图的应用在PCI术后预后评估方面有可能发挥更大作用. 【关键词】　冠状动脉粥样硬化性心脏病;三维超声心动图;经皮冠状动脉介入术;左心室收缩功能;左心室收缩同步性Quantitativeanalysisofmyocardialmovementandleftventricularfunctionwiththreedimensionalechocardiographyinpatientswithcoronaryheartdiseasebeforeandafterpercutaneouscoronaryintervention【Abstract】Objective　Toexploretheapplicationvalueofreal－timethree－dimensionalechocardiographyinpatientswithcoronaryatheroscleroticheartdisＧease(CHD)afterinterventionaltreatment．Methods　１２０casesofpatientswithcoronaryheartdiseasetreatedbypercutaneouscoronaryinterventionand２８０caseswithoutPCIweretreatedastheresearchobject．ThechangesofleftventricularsystolicfunctionandleftventricularsystolicsynchronyindexinpatientswithcoroＧnaryheartdiseaseafterPCIwereanalyzedbythree－dimensionalechocardiography．Results　Theleftventricularenddiastolicvolume(LVEDV),endsystolicvolＧume(LVESV),leftventricularejectionfraction(P＞０．０５)werenotsignificantlydifferentbetweenPCIgroupandcontrolgroup(LVESV),andthedifferencewasstatisticallysignificant(P＜０．０５)at６monthsafterPCI,andthedifferencewasstatisticallysignificant(P＜０．０５)at６monthsafterEFPCI(EF)．Conclusion　PCIcaneffectivelyimproveleftventricularfunctionandsystolicsynchronyinpatientswithcoronaryheartdisease,andtheapplicationofthree－dimensionalechocarＧdiogra【Kpheyywmoarydspl】ayamoreimportantroleintheprognosisofpatientswithPCI． coronaryheartdisease;real－timethree－dimensionalechocardiography;percutaneouscoronaryintervention;Leftventricularsystolicfunction;Leftventricularsystolicsynchrony 【中图分类号】R５４０．４ 【文献标识码】B 　　【文章编号】１００８－６３１５(２０１５)１２－１５０２－０１　　 　　本
