精品论文 参考文献 探讨高血压肾病早期肾损伤联合检测同型半胱氨酸和胱抑素C的临床价值 李丽 丁良辰 　　（沾化县人民医院检验科 山东沾化 256800） 　　【摘要】目的：探讨高血压肾病早期肾损伤联合检测同型半胱氨酸和胱抑素C的临床价值。方法:选择2013年1月至2015年10月诊为高血压肾病的患者92例组成实验组，选择同期健康体检者30例设为对照组，同时测定同型半胱氨酸和胱抑素C的浓度水平, 综合分析结果。结果:实验组患者的同型半胱氨酸和胱抑素C浓度水平明显高于对照组，差异具有统计学意义（P＜0.05），且随着病程增长，同型半胱氨酸和胱抑素C浓度水平均呈现上升趋势；同型半胱氨酸和胱抑素C的敏感性均超过80.00%，早期组患者联合检测同型半胱氨酸和胱抑素C的敏感性明显高于同期单项检测结果，差异具有统计学意义（P＜0.05），特异性有所降低，但中期组和晚期组无明显差异（P＞0.05）.结论：同型半胱氨酸和胱抑素C都是辅助诊断高血压肾病的有效指标，且对于疾病的监测和愈后也有一定价值，联合检测能够有效提高早期肾损伤的敏感性，降低漏诊风险。 　　【关键词】高血压肾病；同型半胱氨酸；胱抑素C 　　【中图分类号】R544.1 【文献标识码】A 【文章编号】1007-8231（2015）22-0044-02 　　To explore the clinical significance of the early renal damage of the hypertensive nephropathy diagnosis by the Homocysteine and Cystatin C joint determination Li Li, Ding Liangchen. Methods Choosing 92 cases of patients with the hypertensive nephropathy for the experimental group and 30 cases healthy check-up for the control group, testing the plasma Homocysteine and the Cystatin C, analyze the results comprehensively. Results There were significant much Higher of the Homocysteine and the Cystatin C of the experimental group than the control group(P＜0.05), and with the course of growth, homocysteine and cystatin C levels showed a rising trend. The positive rate of the Homocysteine and the Cystatin C were more than 80%, there were significant much Higher of the Homocysteine and the Cystatin C joint determination of the early group than Separate detection(P＞0.05), the specificity decreased, but there were no significant differences of the metaphase group and advanced group.
