精品论文 参考文献 斑马导丝在打折鼻胆管冲洗的应用价值 大连大学附属中山医院 胆道微创外科 辽宁大连 116001 　　摘要：目的：探讨斑马导丝在ENBD术后打折鼻胆管冲洗的应用。方法：回顾性分析2012年8月至2014年8月ENBD术后，将68例打折鼻胆管患者分为观察组（34例）和对照组（34例），对照组实施传统鼻胆管外接注射器鼻胆管冲洗，观察组采用斑马导丝植入鼻胆管内，斑马导丝末端经注射器乳头置入注射器针管内进行鼻胆管冲洗，最后对两组进行冲洗效果及患者满意度调查。结果：观察组采取斑马导丝置入鼻胆管内接注射器鼻胆管冲洗效果及满意度明显高于对照组。结论：ENBD术后打折鼻胆管应用斑马导丝植入鼻胆管内接注射器能较好地保持引流的通畅性，能明确胆汁引流是否通畅，控制感染，降低临床操作难度、减轻护理强度，实用性强，有较高的临床价值，值得推广应用。 　　关键词：斑马导丝；鼻胆管冲洗；护理 　　[Abstract]Objective：To explore the application of zebra guide wire in the broken nasal bile duct rinse after ENBD surgery.Methods：A retrospective analysis from August 2012 to August 2014 after ENBD surgery，68 patients with broken nasal bile duct were divided into observation group（34 cases）and control group（34 cases），the control group in the implementation of traditional nose bile duct external syringe nose bile duct flushing，The observation group using the zebra guide wire into the nasobiliary duct，the end of the zebra guide wire into the syringe through the nipple into the syringe needle tube for nasal bile duct rinse，and finally the two groups for flushing effect and patient satisfaction survey.Results：In the observation group，the effect and satisfaction of the nasal bile ducts implanted into the nasal bile duct with the zebra guide wire was significantly higher than that of the control group.Conclusion：After ENBD surgery，the application of broken nasal bile duct zebra guide tube implantation of nasal bile duct internal syringe can better maintain the drainage of patency，can clear whether the bile drainage unobstructed，control infection，reduce the intensity of clinical operation difficulty，reduce nursing，strong practicability，it has high clinical value，worthy of promotion and application. 　　[Key words]Zebra guide wire；Nasal bile duct rinse；Nursing 　　 　　 　　近年内镜技术的不断发展，内镜下鼻胆管引流术（ENBD）已成为梗阻性黄疸、急性化脓性胆管炎、胆源性胰腺炎等疾病十分有效的治疗方法，ENBD相对简单，损伤小、操作时间短，不需开腹手术，对生理干扰极小，能迅速有效解除胆道梗阻，减压引流，达到治疗目的，使患者平稳度过危重期，避免了手术风险[1]，该技术被广泛临床应用，但鼻胆管质地较软，管径较细小，引流过程中容易出现打折、弯曲、成角、阻塞、受压等异常情况，容易被胆汁中的胆泥、碎石、脓性分泌物阻
