精品论文 参考文献 新型便盆垫用于髋部骨折卧床患者排便的效果 江雪莲1 梁金仙2通讯作者 张 毅1 胡桂平1 郭晓玲1 黄 莺1 　　1.十堰市太和医院/湖北医药学院附属医院 湖北十堰 442000； 　　2.郧西县人民医院 湖北十堰 442600 　　【摘 要】目的：研制一种新型便盆垫用于骨折卧床患者排便,并从疼痛程度、排便情况、评价该便盆垫的安全性和有效性。方法：选取2014年1至11月在湖北医院学院附属太和医院骨科住院的年龄大于50岁的髋部骨折需床上大便的患者200例,随机分为研究组和对照组各 100 例。研究组应用自行研制的新型便盆垫用于需床上大便的患者,对照组仍采用传统的徒手抬臀法将患者臀部抬起，放置便盆排便。观察在排便过程中因为姿势的改变导致患者骨折处疼痛的评分。并采用问卷调查的方法调查2组患者发生排便困难及便秘的原因。结果：研究组和对照组患者在排便过程中的疼痛评分分别为(4.7plusmn;0.9)和(8.6plusmn;0.9)分，差异有统计学意义(t=-9.978，Plt;0.01)。研究组患者在使用新型便盆垫后发生排便困难较对照组降低(Pgt;0.05)。结论：该新型便盆垫是一种简单有效的协助卧床患者排便的工具，减少了髋部骨折患者因疼痛因素、环境因素、排便习惯因素而导致的排便困难，减少了该类患者便秘的发生。 　　【关键词】骨折；卧床；排便；便秘；疼痛 　　【中图分类号】R473 【文献标识码】B 【文章编号】1674-8999（2015）8-0117-02 　　【Abstract】Objective: to develop a new type bedpan cushion for fracture bedridden patients defecation, safety and effectiveness and the degree of pain, defecation, the evaluation of the toilet pad.Methods: from 2014 to November at the age of 1 hospitalized in the Department of orthopedics,Taihe Hospital Affiliated Hospital of Hubei College of more than 50 year old hip 200 cases need bed stool fracture, were randomly divided into study group and control group with 100 cases in each group. Study on novel bedpan cushion group applied a kind of self-made bed stool for patients, the control group using the traditional method of manual lifting the buttocks with the buttocks,placed a bowel movement.Observation during defecation for posture change cause a fracture pain score. And by adopting the method of questionnaire survey of 2 groups of patients with difficult defecation and constipation.Results: the study group and the control of pain during defecation of the patients scores were (4.7+0.9) and (8.6+0.9),the difference was statistically significant (t=-9.978, Plt;0.01).
