* Skin temperature under a PRE (patient return electrode) will increase due to the heat produced by ES (electrosurgical) current returning through the PRE to the ES generator. The temperature increase is in direct relationship to the ES power settings and length of instrument activation. Valleylab has performed extensive testing to ensure that, under normal to extreme circumstances, electrosurgical current produced during traditional electrosurgical procedures is safely returned to the ESU without increasing the temperature under the PRE to a potentially dangerous level (112? F). It is sometimes noted that the skin under the PRE may be “pink”. This may be due to the heat produced by the electrical current or it could be due to patient sensitivity to adhesive. Patient skin temperature is approximately 93 ?F when a patient’s core temperature is 98.6?F. As the body temperature increases, the skin becomes pink. An example of this is the pink skin resulting from the application of a heating pad. The “pink” will usually disappear within 30 minutes whether from electrosurgery or a heating pad. Should the skin temperature rise above 112? F, skin injury occurs no matter what the cause. Several patient injuries have been reported to the FDA relating to a “forced air warming device” such as the Bair Hugger? but these were not directly linked to electrosurgery. The following is an excerpt written by Ms. Marders, a nurse consultant for FDA: “Complications of forced air warming devices (such as the Bair Hugger?) have been reported when a blanket is not attached to the hose.
