精品论文 参考文献 肝内外胆管结石术前留置复尔凯鼻胃管效果观察 毛红梅 谭月英 徐 瑾 陈月娟 徐镘棋（云南省昆明医学院第二附属医院 云南昆明 650101) 【中图分类号】R657.4+2【文献标识码】A【文章编号】1672-5085（2012）28-0145-02 【摘要】目的 探讨 肝内外胆管结石引起阻塞性黄胆行胆囊切除术、胆道探查术，术前放置鼻胃管的重要性和必要性。方法 将110例择期行胆囊切除术、胆道探查术患者随机分为两组, 对照组在麻醉前置普通鼻胃管。观察组麻醉前放置复尔凯鼻胃管。比较两组患者放置前后身体适应状况和肠内营养应用的效果评价。结果 观察组与对照组比较, 置管成功率高，咽喉刺激症状轻，恶心呕吐症状小，肠内营养应用效果好。结论 选择放置复尔凯鼻胃管, 患者刺激损伤较小,可减轻痛苦，顺利完成肠内营养治疗，护理人员更易操作。 【关键词】胆囊切除术 胆道探查术 复尔凯鼻胃管 肠内营养 【Abstract】Objective Discuss the importance and the necessity of placing nasogastric tube beforeoperations such as cholecystectomy, bile duct exploration operations on obstructivejaundice that was caused by calculus of bile duct inside and outside liver.Methods 1) Divide 110 cases of cholecystectomy and bile duct explorations into twogroups randomly; 2)Place ordinary nasogastric tube before anesthesia for the conditional control group while place Flocare nasogastric tube before anesthesia for the experimental group; 3)Asses and compare the physical adaption as well as enteral nutrition application effects before and after the placing of nasogastric tube for thepatients in each group.Result By the comparison of experimental group and conditional control group, we concluded that the experimental group generated better results in terms of higher rate of successful placing, milder throat irritation symptoms, less nausea and vomiting, better enteral nutrition application effects.Conclusion The benefits of placing Flocare nasogastric tube are relatively minor stimulation and injury to patients thus reducing suffering.In addition, it could also bring about smoother treatment of enteral nutrition, and easier operation for nursing staff. 【Key words】Cholecystectomy Bile duct exploration Flocare nasogastric Enteral nutrition 胆管结石或胆囊结石石头阻塞了胆管，造成胆汁无法排入十二指肠，皮肤呈暗黄或绿褐色。因胆道阻塞，胆汁不能进入肠道而粪色变淡或呈陶土色，尿胆原减少或缺如。胆汁中的各种消化酶不能正常吸收使患者饮食情况差，进食高营养饮食后出现消化不良、腹胀、腹痛、恶心呕吐造成患者电解质紊乱，长期清淡素食导致患者消瘦、营养不良严重者出现低蛋白血症，这些因素影响术后体质恢复和伤口愈合。术前放置鼻胃管作为胆道常规术前准备,目的是为了胃肠减压及术后胃肠功能，没
