精品论文 参考文献 脑外伤后精神障碍患者的护理体会 叶亚芳 　　（江苏省无锡市锡山人民医院神经外科 214011） 　　【摘要】目的：通过总结对颅脑外伤后精神障碍患者的相关护理经验和体会，进一步规范护理措施，提高护理质量，以有效控制和改善患者的精神症状。方法：通过对医院5年来的81例颅脑外伤后精神障碍患者出现的各种急性精神障碍和慢性精神障碍症状，采用全面的综合性护理方法，可以使大多数精神症状随着脑损伤的恢复，症状得到缓解好转。结果：本组痊愈31例，显著进步35例，进步8例，效果满意。结论：对颅脑外伤后精神障碍患者实施综合性护理，并加以个性化护理干预，可有效改善患者精神症状，减少并发症的发生，促进康复。 　　【关键词】颅脑损伤 精神障碍 护理 　　【中图分类号】R473.74 【文献标识码】B 【文章编号】2095-1752（2013）36-0331-02 　　【Abstract】Objective: To standardize the nurse care measures, improve nurse care quality and control the patientsrsquo; psychiatric symptoms through the summary of related nurse care experience for the patients with psychiatric symptoms due to craniocerebral trauma. Methods: A total of 81 cases with psychiatric symptoms due to craniocerebral trauma got various acute and chronic psychiatric symptoms in recent 5 years in hospital. The comprehensive nurse care measures were applied in order to make the psychiatric symptoms achieve remission with the recovery of craniocerebral trauma. Results: Total 31 cases were cured successfully and other 35 cases were improved significantly. The reminder 8 cases got limited improvement. Conclusions: The patients with psychiatric symptoms due to craniocerebral trauma can get symptoms improvements and complication reduction through the comprehensive and individuation nurse care. 　　【Key words】Craniocerebral trauma psychiatric symptoms Nurse care 　　近年来，随着交通事故等意外事件的频发，颅脑外伤患者日益增多，颅脑外伤后精神障碍患者也随之呈逐年上升趋势。 　　颅脑损伤所致精神障碍是指颅脑受到外力的直接或间接作用，引起脑器质性或功能性障碍时出现的精神异常。脑外伤后的精神障碍十分普遍，这可能是由心理社会因素及外伤性器质因素共同发挥作用所致[1]。本病在颅脑损伤患者中占0.05%～0.15%[2]，在以颅脑损伤为主要收治对象的医院脑外科较多见。无锡市锡山人民医院脑外科2008年1月至2012年12月共收治颅脑外伤后精神障碍患者81例，其主要临床表现分为急性精神障碍：脑震荡，谵妄；慢性精神障碍：智力下降、记忆障碍、情感障碍、人格改变、脑震荡后综合症等症状，并常伴有一系列并发症，如感觉障碍，运动障碍、昏迷等。 　　医院脑外科病区护理组在开展综合性护理的基础上，严格操作规程，针对患者的个体差异实施预见性护理[3]，取得了良好的护理效果。现报告如下。 　　1.临床资料 　　1.1 一般资料 　　本组患者81例，男69例，女12例，年龄12～68岁，平均（32.4plusmn;22.1）岁，交通事故58例，摔跌12例，殴打8例，空中重物击伤3例。 　　1.2 临床表现 　　伤后63例患者出现昏迷，昏迷时间30分钟～1个月。临床诊断为重型颅脑损伤39例，CT提示硬膜外血肿
