精品论文 参考文献 药品调剂错误的原因及处理措施分析 河南省人民医院 河南 郑州 450003 [摘要]目的：药剂师是病人能够安全及有效使用药品的最主要负责人员，因此，若是药剂师发生错误是非常的危险。本文将对医院药剂师药品调剂错误的原因进行分析，并提出处理措施。方法：选取2013年3月到2015年3我院药剂师226张调配差错处方进行原因分析。结果：其中，226张差错处方均在发药过程中被发现。处方中调配差错的类型主要有少调115张（50.88%）、多调25张（11.06%）和错调86张（38.05%）。具体来说，包括药品错误、规格或数量错误、不按规定使用处方(麻、精神一类、精神二类及儿科处方)、漏发、调配不完整、发药交代不清(特指用法及保存)等。结论：我院药剂师应加强对药品调剂的重视，提高自己的业务能力和水平，严格按照规范，不增加、不减少、不错误调配药品，确保患者用药安全。 关键词：药剂师；药品调剂；错误；处理措施 Analysis on the Causes of the Hospital Pharmacy Drug Dispensing Error and its Treatment Measures Zhao Fei, ChaidongYan,Qi Yanxia Zuo zhangyan Henan Provincial Peoples Hospital of Henan Zhengzhou 450003, China [Abstract] Objective: Pharmacists is one of the most patients can safely and effectively use drugs are mainly responsible for personnel, as a result, there was an error if the pharmacist is very dangerous.
