提 要 目的：观察并探讨加味补阳还五汤对2 型糖尿病患者下肢血管病变(Lower Extremity Arterial Disease of Diabetes Patients ;LEADDP) 的临床疗效。方法：选择符合 气阳两虚证入选标准的2型糖尿病下肢血管病变患者病例60例，随机分为治疗组与对 照组各30例。两组均坚持基础治疗：包括控制饮食，适度运动、口服降糖药物或胰岛 素治疗，口服阿托伐他汀每日10mg，阿司匹林肠溶片每日100mg治疗,在此基础上， 治疗组加用加味补阳还五汤，疗程为2月，进行系统临床研究。着重观察治疗前后患 者的主要临床症状及体征，治疗前后血小板聚集率(PAgT)、血内皮素-1 （ET-1 ）、FBG 、 和HbAlc 、TG、TC、LDL-c 的变化。结果：经临床观察,对照组在对改善2型糖尿病合 并下肢血管病变临床症状、症候积分方面均优于对照组(P0.01或P0.05);在降低PAgT 及ET-1方面组优于对照组（P0.01 ）;在改善血FBG 、HbAlc 、TG、TC、LDL-c方面与 对照组无明显差异。结论：本次临床观察发现补阳还五汤明显改善了2型糖尿病下肢 血管病变患者的临床症状，降低血小板聚集率，并能减少炎性介质ET-1 。但在降低血 糖，调节血脂水平方面，与对照组相比无明显优势。显示了加味补阳还五汤对2型糖 尿病下肢血管病变患者有良好的治疗效果。 关键词 加味补阳还五汤；2 型糖尿病合并下肢血管病变；气阳两虚证 Clinical study on Modified Buyang Huanwu Decoction Treating Lower Extremity Arterial Disease of Diabetes Specialty: Traditional Chinese medicine intergrated western medicine Author: Chen Xiangyan Tutor: Zhao Quanlin Abstract Objective: To observe and investigate the effect of treating lower extremity arterial disease in patients of type 2 diabetes （ LEADDP ） with modified buyang huanwu decoction.
