PFN内固定与半髋关节置换治疗高龄不稳定型股骨粗隆间骨折的疗效对比 　　【摘要】 目的：?^察比较PFN内固定与半髋关节置换治疗高龄不稳定型股骨粗隆间骨折的效果。方法：选择2014年2月-2015年4月在本院治疗的86例高龄不稳定型股骨粗隆间骨折患者作为研究对象，根据不同的治疗方式将其分为研究组和对照组，各43例。对照组患者采取PFN内固定术治疗，研究组患者采取半髋关节置换术治疗，对两组患者的治疗效果进行分析比较。结果：治疗后，两组患者手术时间比较，差异无统计学意义（P0.05）；对照组患者的切口长度、术中出血量、输血量、术后引流量均明显优于研究组，差异均有统计学意义（P0.05）。结论：对高龄不稳定型股骨粗隆间骨折的患者采取PFN内固定术治疗，具有切口长度短、术中出血量少、输血量少、术后引流量少等优点，采取半髋关节置换术治疗，具有术后卧床时间短、患肢负重时间短、术后内科并发症发生率及置物并发症发生率低等优点，两种方法均能有效的治疗高龄不稳定型股骨粗隆间骨折，需根据患者的实际情况选择手术方式。 　　【关键词】 PFN内固定； 半髋关节置换； 高龄； 不稳定型股骨粗隆间骨折 　　【Abstract】 Objective：To observe and compare the clinical effect of PFN internal fixation and half hip replacement in treatment of unstable intertrochanteric fractures in elderly patients.Method：86 elderly patients with unstable intertrochanteric fracture in our hospital from February 2014 to April 2015 were selected as the research objects，according to different treatment methods，they were divided into study group and control group，43 cases in each group.The patients in control group were treated with PFN internal fixation，while patients in study group were treated with half hip replacement，the treatment effects of two groups were analyzed and compared.Result：After treatment， the operation time of the two groups were compared，the difference was no statistically significant（P0.05）；the incision length，intraoperative blood loss，blood transfusion volume，postoperative drainage volume in control group were significantly better than those of the study group，the differences were statistically significant（P0.05）.Conclusion：The PFN internal fixation for the treatment of unstable intertrochanteric fractures in elderly patients，which has advantages of short length of incision，less bleeding，less blood transfusion，less postoperative drainage，and the treatment of half hip replacement which has advantages of short lying in bed time and short weight-bearing limb time，low medical complication rate and low complication rate after operation，therefore，two methods can be effective in the treatment
