医学论文-YL1型颅内血肿穿刺针的临床应用 【摘要】? 目的 探讨YL1型颅内血肿穿刺针对颅内疾病微创治疗的价值及临床应用。方法 回顾性分析本院2005年3月至2007年3月应用YL1型颅内血肿穿刺针治疗颅内疾病患者58例（高血压幕上脑出血38例；慢性硬膜下血肿12例；急性硬膜外血肿5例；脑积水2例；颅内张力性积气1例）评价YL1型颅内血肿穿刺针其在颅内疾病的应用价值。结果 经采用CT定位YL1型颅内血肿穿刺针微创手术治疗颅内疾病患者58例，存活51例；死亡4例；术后放弃治疗3例。结论 采用YL1型颅内血肿穿刺针微创手术治疗颅内血肿、积水、积气是简单、有效的微创手术方法。 【关键词】? 微创外科；颅内血肿；颅内穿刺针 ??? Abstract: Objective?? To investigate the value and clinical problems of YL1 intracranial hematoma crasher in the minimally invasive surgery treatment of intracranial disease. Methods?? Totally 38 cases of hypertension supratentorial cerebral hemorrhage, 12 cases of chronic subdural hematoma, 5 cases of acute epidural hematoma, 2 cases of hydrocephalus and 1 case of tension pneumocephalus were treated with YL1 intracranial hematoma crasher from March 2005 to March 2007. The clinical date was retrospectively analyzed and the therapeutic efficacy of YL1 intracranial hematoma crasher was evaluated. Results?? Fiftyone cases were cured, 4 cases were dead and 3 cases gave up treatment after operation. Conclusion?? The minimall invasive treatment of intracranial hematoma, hydrocephalus and pneumocephalus with YL1 intracranial hematoma crasher is simple and safe in operation with satisfied effects. ??? Keywords: minimally invasive surgery; intracranial hematoma; intracal puncture needle ??? 高血压幕上脑出血、慢性硬膜下血肿、急性硬膜外血肿、脑积水、积气是颅脑外科的常见疾病，治疗方法多种多样，传统手术治疗多采用开颅血肿清除术，术后死亡率和致残率仍比较高［12］。本院于2005年3月至2007年3月收治颅内血肿穿刺针治疗颅内血肿、积水、积气患者58例，采用CT定位YL1型颅内血肿穿刺针微创手术治疗，取得满意疗效，现报道如下。 ??? 1? 资料与方法 ??? 1.1? 临床资料 ??? 本组病例58例，其中男40例，女18例；年龄18～76岁，平均53岁。高血压幕上脑出血38例，高血压???史1～21年，平均12年，脑出血发病到穿刺治疗2～72 h；高血压幕上脑出血组：清醒3例，浅昏迷19例，中度以上昏迷16例，偏瘫34例，一侧瞳孔散大20例，双侧瞳孔散大3例。CT检查情况:基底节区出血31例，伴破入脑室13例，皮层下出血7例，出血量30～170 ml，平均55 ml，发病至手术时间2～72 h。慢性硬膜下血肿组：神志均清醒，一侧肢体肌力下降8例，无肢体肌力下降4例。急性硬膜外血肿组：神志均清醒，一侧肢体肌力下降1例，无肢体肌力下降4例。脑积水2例，为小脑扁桃体下疝术后2周1例，脑出血后3个月1例。张力性气颅1例，为经鼻蝶垂体瘤切除术后3周。 ??? 1.2? 手术方法 ??? 微创手术方法:①血肿：入院后经CT检查，戴头皮标记CT定位穿刺点与穿刺方向，避开头皮，脑膜及侧裂血管和大脑主要功能区，YL1型颅内血肿穿刺针穿刺。根据颅内情况，采用单针或多针穿刺引流。局麻，0.5 cm小切口达颅骨，穿刺针钻颅、换芯、推
