医学论文-Sky膨胀式椎体成形器在骨质疏松性椎体压缩骨折重建中的初步应用 ???????????? 作者：王文军 王麓山　胡文凯　刘利乐　姚女兆　王程 【关键词】? 骨质疏松 ??? 摘要：［目的］介绍一种用于重建T5~L5间的椎体压缩性骨折的新方法。［方法］12例（19椎）骨质疏松性椎体压缩骨折患者，应用Sky膨胀式椎体成形器进行椎体扩张,使用PAMMA作为填充物重建骨折椎体，比较手术前后疼痛缓解程度（VAS法）的变化与Cobbs角的变化。［结果］本组病例均获随访2~8个月，平均6个月，无1例死亡，1例出现椎管内渗漏,无神经缺失症状，均达到疼痛缓解,手术前后Cobbs角有显著差异。［结论］Sky方法具有满意的止痛效果，还能较好的纠正后突畸形，有效恢复并保持椎体高度。而且可减少患者的经济负担，操作简单，值得临床推广。 　　关键词：骨质疏松性椎体压缩骨折;经皮椎体成形术; 骨水泥 　　Reconstruction of osteoporotic vertebral compression fracture by Sky bone expander system 　　Abstract：［Objective］To introduce a new method for treating oteoporotic vertebral compression rracture from T5 to L5.［Method］Twelve cases (19 vertebrae) suffering vertebral compression fractures were treated by Sky bone expander system which had expanded vertebral bodies and reconstructed the vertebral body with PAMMA cement. The clinical effect was evaluated by observing the changing of visual analog scale (VAS) and the Cobbs angle.［Result］All cases were followed up from 2 to 8 months (averaged, 6 months). The postoperative? Cobbs angle has statistically decreased. Leakage of PAMMA into spinal canal occurred in one case without neurological deficit. Pain relieved obviously in all cases.［Conclusion］Sky bone expander system provids significant pain relief effect in the cases of oteoporotic vertebral compression fractures, and can improve the kyphosis? deformity and remain vertebral height. 　　Key words Oteoporotic vertebral compression fracture;Percutaneous vertebroplasty;PAMMA 　　在影像设备的帮助下经皮椎体成形术（percutaneous vertelroplasty，PVP）和 PKP（percutaneous kyphoplasty）已经成为治疗老年人骨质疏松性椎体压缩骨折安全有效方法［1,2,3］该项技术除治疗骨质疏松引起的椎体压缩性骨折外，对椎体血管瘤、椎体恶性肿瘤（包括椎体转移瘤）也有很好的疗效。但PVP 对脊柱后凸畸形作用差，PKP也存在对严重后凸畸形椎体前中柱重建效果不理想且操作复杂，价格昂贵的缺点，不易推广。本科从2004年10月~2005年3月使用一种新式的Sky膨胀式椎体成形器完成骨质疏松性椎体压缩骨折重建12例(19椎)。取得了良好的临床疗效。 　　1? 资料和方法 　　11 一般资料 　　本组12例（19椎）, 男8例，女11例；年龄44~72岁，平均 663 岁；受累椎体T5 1例, T5 T11 L4 1例, T9 L3 1 例, T9 L2 1 例,T10,T11 1 例, T12 L3 1 例, L1 2 例, L1 L4 1 例,L2 1 例, L3 1 例,??
