With the shoulder and elbow flexed at 90o, a forced internal rotation of the shoulder is carried out. The internal rotation would produce pain if there were a conflict of space or a subacromial entrapment. This test seems to be the most sensitive of the tests for evaluating impingement (92 %). 幻灯片12 补充检查 Lift-off test(抬离试验) 患者将手背置于下背部手心向后，嘱患者将手抬离背部（必要时给予阻力），不能完成动作为阳性，提示肩胛下肌损伤。 幻灯片13 Neer征 肩极度内旋，肩胛骨平面前屈上举，当出现疼痛时，将肩外旋继续上抬，疼痛减轻或消失为阳性（主动） 检查者用手向下压迫患者患侧肩胛骨并使患臂上举，如因肱骨大结节与肩峰撞击而出现疼痛即为撞击试验阳性（被动） 幻灯片14 拿破仑试验（belly-press test 压腹试验） 患者将手置于腹部，手背向前，屈肘90度，注意肘关节不要贴近身体。检查者手向前拉，嘱患者抗阻力做压腹部动作。两侧对比，阳性者力量减弱。阳性提示肩胛下肌(肩关节内旋肌)损伤。 幻灯片15 Yocum’ s Test With the hand over the contralateral shoulder, the patient is asked to elevate his/her elbow. The presence of pain in the periacromial area is indicative of a problem of subacromial impingement.
