Materials and Methods We scanned the brains of 3950 in-service aviators by MRI/MRA, and analyzed the brain parenchyma and cerebrovascular distribution. [Results]: Among the range of 3950 in-service pilots, 53 of them had brain diseases which were not included in the disease spectrum in the ‘civil aviators’ health check-up regulations’ (CCAR-67-R2). 24 cases applied for the special permission of aviation license. 19 cases succeeded, and 5 cases failed. The brain disease spectrum was comprised of the four types: 30 cases of cerebrovascular disease, 7 cases of brain parenchyma non-tumor change, 3 cases of brain parenchyma tumor lesion, and 13 cases of intracranial non-original tumors or occupying lesions. [Conclusions]: Magnetic resonance imaging examination revealed the brain disease spectrum in pilots, thus offering data to decision-making in risk management of civil flying. Key words: aviator; flying incompetence; MRI; MRA 引 言 飞行失能：指飞行员在空中因为自身问题（主要是疾病或心理问题）不能继续执行飞行任务的状况,也称“空中失能”。2007年11月香港/2012年2月郑州的飞行事件均匀由于飞机驾驶机长癫痫发作，导致空中飞行失能引起。所幸事故航班为双机长配置，当班副机长经验丰富，杜绝了两起重大飞行事故的发生。 Flight disability: refers to the pilot due to their own problems mainly caused by disease or psychological problems that can not continue to perform a flight mission, also known as air disability. 2007.11HK2012.2ZhengZhou flight incident were all caused by captain’s epilepsy stroke.
