TheresearchprogressofrelationshipbetweenPAPP-AandcoronaryheartdiseaseAbstractBackground:Coronaryheartdisease（CHD）hasseriouslyharmedthehumanhealth.Recentresearchesshowthatinflammatoryreactionplaysaimportantroleintheforminganddevelopmentofatherosclerosis,withvariousinflammatoryfactorsandmacrophage.OneoftheinflammatoryfactorbeingstudiedhotlyisPAPP-A(pregnancy-associatedplasmaprotein-A).ItisfoundedthatPAPP-AhasasignificantroleinVP(vulnerableplaque)forminganddeveloping,anditisVPthatdirectlyinducesACS（acutecoronarysyndrome）.SotherearealotofstudiesontherelationshipbetweenPAPP-AandACS.Objective:ThroughstudiesonthestructureandfunctionofPAPP-AandIGFaxis,wediscusstherelationshipbetweenPAPP-Aandatherosclerosis,andmakefurtherclinicalresearchesonPAPP-Aapplicationvalueindiagnosis，therapyandprognosisevaluationofCHD.Methods:RetrievetheliteraturesandsummarizetheprogressinrecentyearsrelatingtothestructureandfunctionofPAPP-A,therelationshipbetweenthelevelofPAPP-AandvulnerableplagueandtherelationshipbetweenthelevelofPAPP-AandCHDinWanfang,WeipuandPubmeddatabase.Results:PAPP-Aisonememberofmetalloproteinasefamily,andcandirectlyerodetheplaque,inducingmorethin-capfibroatheromasandplaqueruptures;attheotherside,PAPP-AenhancesinflammatoryreactionthroughIGFaxisandindirectlydecreaseplaquestability.PAPP-Aisvaluableinpredicitingmajoradversecardiacevents、classifyingriskofCHDandpredictingtheriskofno-reflowafterPCI.AnditisfoundedthatPAPP-As、CD40LandCRPparticipateformationofVPtogether.Conclusion:PAPP-Ahasasignificantroleinatherosclerosisforminganddeveloping,especiallyinVP.PAPP-Ahassomecertainclinicalvalueindiagnosis,therapyandprognosisevaluationofCHD.【Keywords】：PAPP-A,Vulnerableplaque,Coronaryheartdisease正文易损斑块（vulnerableplaque,VP）破裂是急性冠脉事件发生的病理基础，探索新的血清标记物对于VP的早期识别和干预至关重要。新近研究发现妊娠相关血浆蛋白A（pregnancy-associatedplasmaprotein-A,PAPP-A）不仅参与动脉粥样斑块的发生发展，而且在破损斑块或侵蚀斑块的表达明显高于稳定斑块[1]。急性冠脉综合征患者血浆中PAPP-A水平显著高于稳定型心绞痛患者，而且与该类人群发生不良心血管事件有关[2]。因此认为PAPP-A不仅能早期识别易损斑块，而且能预测冠心病患者不良心血管事件的发生，有助于早期干预及治疗，从而减低高危CHD患者的死亡率。1PAPP-
