交锁髓内针和加压钢板对胫骨中远段骨折疗效对比 　　【摘要】 目的 探讨交锁髓内针和加压钢板对胫骨中远段骨折疗效差异。方法 回顾分析胫骨中远段闭合骨折及Ⅰ型和Ⅱ型骨折共28例的临床资料,将其术后疗效进行对比,利用SPSS12.0软件对两组数据进行统计学分析。结果 交锁髓内针组愈合13例,愈合率100%,平均愈合时间为20周(17~32周),膝关节活动度受限1例,踝关节活动度受限2例,跪姿膝前痛(VAS Scores on Kneeling)平均值4.5(0~10),蹲坐姿膝前痛(VAS Scores on Squatting)平均值2.8 (0~9.2)。Jonher-Wruh评分:优8例,良3例,中2例,优良率84.61%。加压钢板组愈合15例,愈合率100%,平均愈合时间为19周(14~28周),膝关节活动受限2例,踝关节活动受限3例,跪姿膝前痛平均值0.7(0~49),蹲坐姿膝前疼痛平均值1.9(0~50)。Jonher-Wruh评分:优9例,良3例,中3例,优良率80%。统计学显示:两组愈合时间无统计学差异P0.05(P=0.23),跪姿膝前痛平均值有统计学差异P0.05。结论 对于胫骨中远段???闭合骨折和Ⅰ型和Ⅱ型骨折,交锁髓内针内固定和加压钢板内固定两种治疗方式的术后近期疗效无明显差异。? 　　【关键词】 胫骨中远段骨折;髓内针;加压钢板;视觉模拟评分法 　　 　　The curative effect contrast of distal tibial shaft fracture between the interlocking intramedullary nail and compressive plate 　　 　　QI Nuo,GUO Jing-dong .The Fist People Hospital of YITONG District, Jilin 130700, China 　　? 　　【Abstract】 Objective To discuss the curative effect difference of distal tibial shaft fracture between the interlocking intramedullary nail and compressive plate. Methods We do a retrospective analysis of the clinical data of 28 cases, include closed fracture 12, Ⅰ type 9,Ⅱtype 7. We do a statistics analysis on the data of two group with SPSS12.0. 　　Results The interlocking intramedullary group:healing rate is 100%, average of healing time is 20 weeks. Limited range of knee motion 1, Limited range of ankle motion 2. the VAS Scores on Kneeling is 4.5, VAS Scores on Squatting is 2.8. The good rate is 84.61%. The compressive plate group:healing rate is 100%, average of healing time is 19 weeks. Limited range of knee motion 2, Limited range of ankle motion 3. The VAS Scores on Kneeling is 0.7, the VAS Scores on Squatting is 1.9. The good rate is 80%. There is a significant difference in the VAS Scores on Kneeling. Conclusion To closed tibial fracture, Ⅰtype fracture and Ⅱtype fracture, the recent curative effect of interlocking intramedullary nail and that of compressive plate have on significant difference.? 　　【Key words】 Distal tibial shaft fracture
