浸润型肺结核并发糖尿病临床探讨 　　【摘要】目的：探讨并发糖尿病的肺结核临床特点及糖尿病控制对结核化疗疗效的影响。方法：收集分析2001年1月～2006年12月，于我院住院的浸润型肺结核并发糖尿病51例的临床资料，并选择同期单纯肺结核569例为对照组进行比较。结果：浸润型肺结核并发糖尿病的患者比单纯肺结核病情较重、干酪病灶多、空洞多、排菌和复治病例比例大的临床特点；在糖尿病稳定，血糖控制在8.4 mmol/L以下，其痰菌阴转率、病灶显吸率与单纯性肺结核的疗效相似。结论：糖尿病与肺结核互有影响，糖尿病对肺结核影响较大，并发糖尿病的肺结核化疗疗效及预后与糖尿病控制情况密切相关。 　　【关键词】肺结核；糖尿病；临床探讨 　　文章编号：1009-5519（2007）11-1605-02 中图分类号：R5 文献标识码：A 　　 　　Clinical analysis of infiltrative pulmonary tuberculosis complicated with diabetes 　　LIU Jian-feng，ZHENG Yuan-yuan，YI Qi-xing 　　(Department of Infectious Diseases,The General Hospital of Tianfu Mining Bureau,Chongqing 400704,China) 　　【Abstract】Objective:To investigate the clinical characteristics of pulmonary tuberculosis complicated with diabetes and the effect of controlling diabetes on the therapeutic effect of tuberculosis chemotherapy.Methods:The clinical date in 51 patients with infiltrative pulmonary tuberculosis complicated with diabetes from January 2001 to December 2006 treated in our hospital were analysed,and compared with those in 569 cases of simple pulmonary tuberculosis as control group.Results:Compared with the simple pulmonary tuberculosis patients,the patients with infiltrative pulmonary tuberculosis complicated with diabetes were more serious,more caseous foci,more pulmonary cava,more bacilli in sputum and more recurrent cases.The therapeutic effect of the patients with infiltrative pulmonary tuberculosis complicated with diabetes was similar to those with simple pulmonary tuberculosis in the sputum bacillus disappearance rate and the tuberculosis focus absorption,if the diabetes was stable and the blood glucose was less than 8.4 mmol/L.Conclusion:Diabetes may impede the recovery of tuberculosis,so controlling the blood glucose is important to the treatment and prognosis of pulmonary tuberculosis complicated with diabetes. 　　【Key words】Pulmonary tuberculosis；Diabetes；Clinical analysis 　　 　　我院感染科于2001年1月～2006年12月间，共收治肺结核并发糖尿病的患者51例，为探讨糖尿病并发肺结核的临床特点及糖尿病控制对结核化疗疗效的影响等，现报道如下。
