中文摘要目的观察加味黄芪建中汤治疗对伴有轻中度焦虑抑郁的脾阳虚证 IBS-D 患者的肠道症状及焦虑、抑郁症状的影响，评价其临床疗效及安全性,并随访观察诱发加重因素，为临床治疗提供指导。方法选取符合纳入标准的我院老年病科门诊 IBS-D 患者 60 例；并随机分为治疗组和对照组，每组 30 例。治疗组给予加味黄芪建中汤治疗，100ml 3 次/日；对照组给予匹维溴铵片，1 片/次，3 次/日，饭后口服，阿米替林片，12.5mg/次， 1 次/日，睡前口服。两组疗程均 4 周。比较治疗前后患者肠道症状和焦虑抑郁症状积分变化情况，根据患者治疗前后肠道症状变化情况进行疗效评价。治疗结束后 4 周对显效及有效患者进行随访观察有无症状加重及加重诱发因素。结果治疗 4 周后，两组药物均能降低肠道症状总积分，治疗组优于对照组（P﹤0.05）；两组药物在腹痛或腹部不适、排便频率、大便性状、腹胀症状积分下降上差异无统计学意义（P0.05），但治疗组在肠鸣、排便急迫感、食欲不振症状积分下降上优于对照组（P﹤0.05）；两组药物均能降低焦虑抑郁积分，但两组差异无统计学意义（P0.05）；对肠道症状进行疗效评价，总有效率 83.3%，优于对照组（66.7%），差异有统计学意义（P﹤0.05）。不良反应：治疗组在治疗期间未出现不良反应；对照组有 3 例患者出现口干症状，4 例患者出现心跳加快。随访：两组共有 16 例患者出现症状加重，其中 8 例因饮食不节诱发，5 例因受凉后诱发，3 例因情志刺激诱发，1 例和月经周期有关。结论 1、本研究表明加味黄芪建中汤能够改善 IBS-D（脾阳虚证）患者肠道症状及焦虑、抑郁症状，对 IBS-D（脾阳虚证）治疗有效，总体疗效优于对照组，安全性高。2、本病诱发加重因素可能和饮食不节、受凉、情志刺激及月经周期有关。关键词加味黄芪建中汤，腹泻型肠易激综合征，脾阳虚证,焦虑和抑郁IAbstractObjective ： To investigate the curative effect of modified Huangqi Jianzhong Decoction on the spleen Yang deficiency syndrome in diarrhea predominant irritable bowel syndrome (IBS-D) patients with mild- moderate anxiety and depressionthrough the observation effect on intestinal anxiety and depression syndrome. And based on the theory mental activities controlled by heart，from the relationship between heart and spleen and stomach to elaborate the relationship between emotionand IBS，to provide theoretical basis for the treatment of IBS-D with emotionalsyndrome，from the perspective of relationship between heart and spleen and stomach.Methods：60 cases fulfilled the Roman Ⅲ criteria of geriatrics outpatients were randomly divided into two groups. Treatment group was treated by modified Huangqi Jianzhong Decoction, 100 ml, 1times/d. Control group were given Pinaveriumbromide, 50 mg, 3times/d, and /w/amitriptyline/amitriptyline12.5 mg 1times/d before sleep.
