成都中医药大学硕士学位论文 成都中医药大学硕士学位论文 - - PAGE 1 - 中文摘要 目的：观察中药内服治疗腰椎间盘突出症的临床疗效。 方法：随机将 80 例腰椎间盘突出症患者分成两组，即用温肾壮筋法 指导中药内服治疗的治疗组和传统的腰椎牵引治疗的对照组（各 40 例）。均于 4 周后观察比较两组患者治疗前后临床症状与体征、疼痛 程度、以及影像学的变化，进行疗效评定。治疗后 3、6、9 个月随访 观察症状复发情况。 结果：（1）治疗组的治愈率、显效率和有效率分别为 37.5%、72.5% 和 95%，而对照组分别为 22.5%、50%和 87.5%，经比较说明治疗组 的总体临床疗效优于对照组（P﹤0.05），且治愈率与显效率均高于对 照组（P﹤0.05）；而有效率比较则无明显差异（P﹥0.05）；治疗组在 缓解症状与体征、疼痛方面疗效优于对照组（P﹤0.05）。（2）治疗后 3 个月两组的复发率分别为 2.63%和 11.43%，无显著性差异（P﹥ 0.05）；治疗后 6、9 个月两组的复发率分别为 7.89%和 25.71%、13.16% 和 40%，有显著性差异（P﹤0.05）。说明治疗组在防止复发、巩固疗 效方面优于对照组。（3）腰椎 CT 片椎间盘突出征象改善情况方面， 两组的改善率分别为 34.37%和 45.71%，两组治疗前、后分别比较无 显著差异（P﹥0.05），两组间改善率比较也无显著性差异（P﹥0.05）。 结论：中药内服是治疗腰椎间盘突出症的一种疗效显著、复发率低、 安全可靠的治疗方法。 关键词：腰椎间盘突出症 温肾壮筋法 临床研究 Abstract Objective：To observe the clinical therapeutic effects of curing the lumbar disc herniation by Chinese herbal oral administration . Methods: 80 cases of lumbar disc herniation patients were divided stochastic into two groups: therapeutic group with 40 cases was treated with the Chinese herbal oral administration and the traditional group with 40 cases was treated with lumbar traction. After 4 weeks treatment, these patients’ clinical symptom and physical sign, pain degrees, and image were compared with before and after treatment and each other to observe the therapeutic effects. Post-treatment 3,6,9 months, to observe the symptom recurrence conditio n after follow-up visit. Results: (1) The cure rate, the marked effective rate and the defective rate were 37.5%,72.5% and 95% in therapeutic group, and they were 22.5%, 50% and 87.5% in control group. The total therapeutic effects, cure rate and marked effective rate of therapeutic group was significantly higher than control group (P0.05). But there was no significant difference in the effective rate between them (P0.05).The therapeutic effects of relieve symptom and physical sign of therapeutic group was significantly higher than control group (P0.05).
