摘要 摘要 i i 摘要 人们的身体健康离不开医疗服务的有效供给，为了实现人人能够享受到医疗服 务，需要医疗服务能够体现出一定的公益性与公平性。我国在 2009 年更是将“提供 安全、有效、方便、价廉的医疗卫生服务”作为规制的总目标。在我国人口老龄化加 速、老龄化与高龄化并存，人类疾病谱逐步向慢性及非传染性疾病转变，生活水平不 断提高等背景下，人们对医疗服务的需求也越来越多。然而，在当前医疗服务市场中 医疗费用却在不断上涨、医疗机构的提供效率也在不断地下降、医疗服务的城乡二元 结构以及盲目就医等医疗问题也愈演愈烈，有效地实现医疗服务的可及性与公平性面 临着愈来愈大的考验。伴随着我国医疗服务市场政府规制的放松以及市场化改革方向 的确立，如何更好地实现医疗服务的规制目标以及政府应该在医疗服务市场中担负怎 样的责任成为本研究的主题。 本文以医疗服务市场政府责任的界定作为出发点，运用卫生经济学与规制经济学 的相关理论对我国医疗服务的产品特征及医疗服务的市场特征进行分析，得出医疗服 务市场存在着垄断、外部效应、信息不对称与供给者诱导需求等市场失灵现象，需要 政府的力量来予以矫正。使用“3E”评价法，结合近年来医疗服务市场中的各项统计 及调查数据，从效果、效率、公平三个角度对规制的成效进行了分析。得出我国医疗 服务市场无论从医疗供给层面还是医疗需求层面都有了较大的改善。然而，医疗服务 市场中仍然存在着医疗费用快速上涨、城乡医疗资源二元化分布、医疗保险基金利用 率低等问题。通过深入分析发现扭曲的价格规制、过高的进入壁垒、不完善的质量规 制以及尚未理顺的管理体制是影响我国医疗服务市场政府规制目标成效的根本因素。 最后，文章在分析比较了以英国、美国、德国以及古巴为代表的医疗服务模式之后， 借鉴国外的先进经验并结合我国的实际情况，分别从价格规制、进入壁垒、质量规制、 管理体制等角度，对我国医疗服务市场政府规制的改革提出相关的对策和建议。 关键词：医疗服务市场；政府规制；“3E”评价法；优化策略 我国医疗服务市场政府规制问题与优化策略研究 我国医疗服务市场政府规制问题与优化策略研究 ii ii ABSTRACT The medical service market has a high degree of information asymmetry, fairness and public welfare characteristics.
