CAR―T细胞治疗在血液系统恶性肿瘤中研究新进展 　　[摘要] 随着社会发展、环境改变等因素，血液系统恶性肿瘤患病人数不断增多，包括淋巴瘤、骨髓瘤、白血病等多种疾病，以往采取化疗等方式治疗，效果并不十分理想。嵌合抗原受体T（CAR-T）细胞治疗属于新型方法，通过对患者输注足量的CAR-T细胞，达到识别肿瘤细胞、靶向杀伤的目的，应用价值较高，且不会出现严重不良后果，安全性、患者耐受性均相对较佳，其以自身免疫细胞对癌变细胞进行清除为基本原理。通过一系列改进，目前以第四代CAR-T细胞治疗为主，其在前几代基础上，加入可选择性的标记，在T细胞修饰、T细胞扩增终止等方面均有其独特优势。但CAR-T细胞疗法目前尚处于Ⅰ期研究，技术并不成熟，且多以B细胞恶性肿瘤疾病为主，在诸多抗原中，主要对CD19进行研究，而对T细胞恶性肿瘤疾病及其他抗原应用效果文献较少，还需扩大样本容量进一步深入探讨。该研究对血液系统恶性肿瘤疾病采取CAR-T细胞治疗的研究进展进行如下综述，便于临床治疗工作的实施，为进一步研究奠定基础。 　　[关键词] 血液系统恶性肿瘤；CAR-T；细胞治疗 　　[中图分类号] R4 [文献标识码] A [文章编号] 1674-0742（2018）03（b）-0192-04 　　New Research Progress of CAR-T Cell Treatment in the Malignant Tumors of Blood System 　　HUANG Zhi-ning 　　Department of Hematology， Baise People’s Hospital， Baise， Guangxi， 533000 China 　　[Abstract] With the social development and environmental changes， the number of malignant tumors in the blood system constantly increases， including the lymphoma， myeloma， leukemia， and the effect of chemotherapy is not ideal， and the infusion of sufficient CAR-T cell of patients can reach the aim of identifying the tumor cells and targeted killing， with higher application value， without serious adverse outcomes， better safety and resistance， currently， the four-generation CAR-T cell by a series of improvement can be the main， it increases the selective markings on the basis of the generations before， which has the unique advantages in the T cell decoration and T cell expansion termination， but the CAR-T cell therapy is still at the stage I study， and the technology is not mature， mostly dominated by the B cell malignant tumor diseases， and in most antigens， the CD19 is the major research object， and the literatures of T cell malignant tumor diseases and application effect of other antigens were fewer， which needs the further study.
