PAGE 摘要 目的：在实习过程中发现在进行静脉穿刺时，不同的进针角度病人的疼痛感也不一样。通过对静脉穿刺进针角度的研究，认为静脉穿刺进针角度以进针时与皮肤成45°~60°角为宜，既不影响穿刺成功率，又可使病人疼痛感明显减轻。 方法：选择住院接受静脉输液意识清晰、能正确表达、准确配合的患者280例,随机分为观察组和对照组,其中男156例,女124例,年龄在20～68岁。实验组及对照组均由作者本人操作，观察组静脉穿刺与皮肤成45°～60°角进针穿过皮肤，对照组以除针头与皮肤成15°～30°角进针，除进针角度不同之外，其余操作实验组与对照组都相同。 结果：观察组静脉穿刺与皮肤成45°～60°角进针，病人疼痛反应轻，其中微痛98例次，一般疼痛30例次，剧痛12例次。微痛率占70％。对照组以与皮肤成15°～30°角进针，疼痛反应明显，其中微痛44例次，一般疼痛62例次，剧痛34例次.其无痛率占31.43％.无痛与一般疼痛之间X2=42.19，P0.01，存在显著性差异，观察组的穿刺成功率为96.4％，对照组是92.1％。患者通过简式McGill疼痛量表所打分值，在0~～13分的观察组为96人，对照组为40人。11～23分的观察组为30人，对照组为68人，24～~37分观察组为14人，对照组32人。 结论：实验表明，静脉穿刺进针角度以进针时与皮肤成45°～60°角时病人的疼痛感明显减轻，且成功率也不受影响。 关键词：静脉穿刺 穿刺角度 疼痛 Abstract Objective: Found during the internship during the venipuncture, the needle point of the different pain patients are not the same.
