Methed: Totally 51 patients of this research all from in Orthopedics of First Peoples Hospital in Chengdu, Random sparate 51 patients to two groups,the Wiltse paraspinal approach groups include 27 patients,the conventional approach groups include 24 patients.The effects were evaluated by operation time, bleeding volume, Incision ,incision pain and dysfunction. Incision pain was measured by visual analog scale (VAS) on the day before operation and 2st day and 10th day after operation.Dysfunction was measured by Oswestry disability index (ODI%) in the 1st month and 6th month after operation. Statistical Analysis the results to conclutions. Results: Results showed that the two groups incision pain No statistically significant in first day preoperative and the Postoperative day,as well as in patients sex and age( p＞0.05 ).However, less bleeding volume and less operation times also include lower Incision pain points what ten days after surgery in Wiltse paraspinal approach groups(p0.05).The lower ODI index that 1st month and 6th month after operation also in Wiltse paraspinal approach groups. Those above mention proved that the Wiltse paraspinal approach operation was effective than conventional approach operation in treatment of Lumbar vertebral fractures. Conclude: Two kinds of approaches are valid ,but treatment of Lumbar vertebral fractures in Wiltse paraspinal approach surgey Concept of minimally invasive.It effective than conventional approach in bleeding volume ,operation times and ODI index.The patients rapid postoperative recovery.
