影响社区脑卒中复发高危因素分析 　　摘 要 目的：分析影响社区脑卒中复发以及复发次数的高危因素，为社区“延伸脑卒中单元”管理提供更多的理论依据。方法：选择2010年至2013年间监测的所辖社区内首发和复发脑卒中患者513例，分为复发组295例和未复发组218例，分析影响脑卒中复发及复发次数的高危因素。结果：影响脑卒中复发的独立危险因素有高血压史、首次发病为脑梗死、冠心病史和糖尿病史，复发组平均具有（2.5±1.5）个高危因素，明显高于非复发组的（2.0±1.5）个（P0.001），高血脂症是影响脑卒中复发次数多少的主要高危因素。结论：高血压、糖尿病、冠心病是社区脑卒中患者复发的高危因素，高血脂症可能是导致复发次数增多的高危因素。 　　关键词 脑卒中 复发 高危因素 　　中图分类号：R743.3 文献标志码：A 文章编号：1006-1533（2016）10-0044-02 　　Analysis of the high risk factors influencing the community stroke recurrence 　　ZHANG Xiaohong， XIAO Jiewen， ZHANG Heping， XU Lihui， LU Ya， MAO Yaqin（Wusong Community Health Service Center of Baoshan District， Shanghai 200940， China） 　　ABSTRACT Objective： To analyze the high risk factors which influenced the community stroke recurrence and recurrence times to provide the more theoretical basis for the community “extended stroke unit” management. Methods： From 2010 to 2013， 513 cases of the first and recurrent stroke monitored under the jurisdiction of the community were selected and divided into a recurrent group with 295 cases， and a non recurrent group with 218 cases. The high risk factors influencing the recurrence and recurrence times of stroke were analyzed. Results： The independent risk factors influencing the recurrence of stroke included the history of hypertension， the first onset of cerebral infarction， the histories of coronary heart disease and diabetes. The patients in the recurrent group had 2.5±1.5 high risk factors on the average， which were significantly higher than 2.0±1.5 in the non recurrent group（P0.001）. Hyperlipidemia was the main high risk factor which influenced the times of the recurrence of cerebral stroke.
