PAGE PAGE VI 2011-8-26 椎体成形术中骨水泥椎间盘渗漏的实验研究中文摘要【目的】探讨椎体成形术中骨水泥椎间盘渗漏后髓核细胞凋亡状态及细胞凋亡调控基因Bcl －2、Bax 、Fas、FasL的表达。【方法】选用 8 只成年杂种犬，每只犬 L2/ 3～L4/ 5 共 3 个椎间盘为实验对象，随机分成对照组、PMMA、CPC 三组。A 组为对照组用 18G 穿刺针仅穿刺椎间盘不注入任何物质；B 组注入 0. 3ml PMMA；C 组注入 0. 3ml CPC 制成骨水泥椎间盘渗漏模型。模型制作 24 周后对 3 组髓核组织行 TUNEL 检测细胞凋亡，并且用免疫组化方法检测椎间盘中细胞的 Bcl －2、Bax 、Fas、FasL 蛋白表达。分别测定 3 组椎间盘组织中的阳性细胞百分率、平均光密度值，3 组资料值分别用方差分析及 SNK 法统计分析。【结果】髓核细胞TUNEL阳性细胞百分率及平均光密度值在对照组、CPC及PMMA 组依次升高；Bax蛋白染色的髓核组织阳性细胞百分率及平均光密度值在对照组、CPC 及PMMA组中依次升高；Bcl - 2蛋白染色的髓核组织阳性细胞百分率及平均光密度值在对照组、CPC及PMMA组依次依次降低；Fas蛋白、FasL蛋白染色的髓核组织阳性细胞百分率及平均光密度值在对照组、CPC及PMMA组依次升高。分别对3组TUNEL、Bax、Bcl - 2、 Fas和FasL染色阳性细胞百分率、平均光密度值行方差分析及SNK法统计分析，每两组间的对应观察指标差异均有统计学意义（P＜0. 05）。【结论】 PMMA、CPC椎间盘渗漏会导致髓核细胞凋亡，Bax、Bcl - 2、 Fas和FasL 蛋白参与了椎间盘细胞凋亡的调节，但凋亡程度不同。【关键词】椎间盘退变；Bcl - 2；Bax；Fas；FasL；光密度；凋亡作者：赵辉指导老师：唐天驷 Experimental S Experimental Study about the Effects of intradiscal Cement leakage in vertebroplasty Abstract Experimental Study about the Effects of Intradiscal Cement Leakage in Vertebroplasty Abstract Part Ⅰ: Establishment of animal model of intradiscal bone cement leakage in Vertebroplasty 【Objective】 To establish a canine vertebroplasty model of intradiscal bone cement leakage that more faithfully mimics the cement leaks into the disc in vertebroplasty of human. 【Methods】 Four lumbar intervertebral discs(L1/2～L4/5) of eight adult dogs (15～ 17kg) were randomly divided into 4 groups. In Group A, the annulus fibrosus of discs were only stabbed by 18G transfixion pin and injected with nothing. In Groups B, the annulus fibrosus of discs were stabbed by 18-gauge needle, and 0.3ml PMMA was injected into them. In Group C, discs were only stabbed by 16G transfixion pin and injected with nothing. In Group D, discs were stabbed by 16-gauge needle and 0.3ml cement was injected into them.
