第三代喉罩用于全身麻醉临床效果研究 　　[摘要] 目的 探讨第三代喉罩（PLMA）应用于全身麻醉的效果。 方法 收集ASAⅠ～Ⅲ级全身麻醉手术患者200例，根据住院时间先后分为观察组与对照组，各100例。观察组应用PLMA，对照组应用气管插管，比较两组的麻醉效果。 结果 观察组围麻醉期血流动力学参数波动幅度显著低于对照组（P0.05）；观察组的并发症发生率为4.0%，显著低于对照组的34.0%（P0.05）。 结论 在全身麻醉中应用PLMA有利于更好地维持血流动力学平衡，减少应激反应与并发症，提高麻醉与手术安全性，值得推广应用。 　　[关键词] 全身麻醉；第三代喉罩；应激反应；血流动力学 　　[中图分类号] R614 [文献标识码] B [文章编号] 2095-0616（2015）14-91-03 　　Research on clinical effect of proseal laryngeal mask airway for general anesthesia 　　QIU Liuji HUANG Huibin JIANG Peng LUO Dexing 　　Department of Anesthesiology，Center Peoples Hospital of Huizhou City Guangdong Province，Huizhou 516001，China 　　[Abstract] Objective To explore the effect of proseal laryngeal mask airway（PLMA）used in general anesthesia. Methods 200 cases of ASAⅠtoⅢ level patients with general anesthesia operation were selected，and were divided into observation group and control group according to the time order of hospitalization，each of 100 cases.The observation group received PLMA，the control group received endotracheal intubation.The effect of anesthesia in the two groups were compared. Results Perioperative hemodynamic parameters fluctuation in the observation group was significantly lower than the control group （P0.05）.The complication rate of the observation group was 4%，which was significantly lower than 34% in the control group（P0.05）. Conclusion The application of PLMA in general anesthesia is conducive to better maintain hemodynamic balance，reduce the stress reaction and complications，improve the anesthesia and operation safety，is worthy of popularization and application. 　　[Key words] General anesthesia；Third generation of laryngeal mask；Stress reaction；Hemodynamics 　　第三代喉罩（PLMA）是在经典喉罩的基础上改良而来，相比于传统喉罩增加了一根对胃管具有封闭与引流作用的管道，套囊更为宽大，能够避免胃内容物反流以及误吸等，从而减少通气穿上，提高安全性和舒适度[1]。我院在全身麻醉手术中应用PLMA替代传统的气管插管全麻，获得了满意效果，现报道如下。 　　1 资料与方法 　　1.1 一般资料 　　收集2013年1月～2014年6月，我院择期行全麻手术者200例，ASAⅠ～Ⅲ级，并发症均在术前得以控制，除外麻醉与手术禁忌者。男98例，女102例，年龄20～80岁，平均（52.1±10.3）岁。其中，132例行电视腹腔镜下胆囊切除术，30例开腹胆囊切除术，26例骨科手术，6例泌尿外科手术，6例妇科手
