高甘油三酯腰围表型与糖代谢异常相关性研究 　　【摘要】211例患者以血甘油三酯≥1.7 mmoL/L、腰围男性≥90 cm或女性≥85 cm为切点，分为nHTWP组、高TG组、WP组和HTWP组。结果表明，HTWP组患者FPG、1hPG、2hPG高于其他三组（均P0.05），但高于WP组和nHTWP组（P0.01），HTWP合并糖代谢紊乱时的TyG指数ROC曲线面积为0.712，结果表明TyG指数对高甘油三酯腰围表型合并糖代谢紊乱的诊断准确性中等。 　　【关键词】高甘油三酯；腰围表型；糖代谢；胰岛素 　　【中图分类号】R587.1；R446.1 【文献标识码】A 【文章编号】ISSN.2095-6681.2018.3..02 　　The study of hypertriglyceridemia-waist phenotype associated with abnormal glucose metabolism 　　LI Kai1， ZHANG Ge2 　　（1.Peoples Hospital of Weiyuan County， Sichuan，Sichuan Neijiang 642450，China；2.Second Affiliated Hospital of Kunming Medical University，Yunnan Kunming 650000，China） 　　【Abstract】According to the cutpoint of plasma triglycerides concentration ≥1.7mmoL/L，waist circumference ≥90cm in man or ≥85cm in woman ，211patiens were divided into four groups：normal waist circumference and triglycerides， normal waist circumference and hypertriglycerides， abdominal obesity and nomal triglycerides， and hypertriglyceridemic-waist group（HTWC）. The results showed that fasting plasma glucose and 1， 2 hour postprandial plasma glucose in hypertriglyceridemia-waist phenotype group were higher than those in hypertriglyceridemia group， waist phenotype group， no-hypertriglyceridemia-waist phenotype group（P0.05）， but higher in hypertriglyceridemia-waist phenotype group than in waist phenotype group， non-hypertriglyceridemia-waist phenotype group（P0.01）.ROC curve area of TyG index in hypertriglyceridemia-waist phenotype associated with disorders of glucose metabolism is 0.712. Result indicate that diagnostic accuracy of TyG index is middle in hypertriglyceridemia-waist phenotype associated with disorders of glucose metabolism. 　　【Key words】Hypertriglyceridemia；Waist phenotype；Glucose Metabolism；Insulin 　　糖代谢异常与心血管疾病关系密切，无论是心血管疾病的患者还是其高危人群，糖代?x异常明显增加心血管事件的危险性[1]，Kahn等[2]报道腰围增加和高TG人群的空腹血糖水平都比腰围和TG正常的人群高。通过研究TyG指数（triglycerides×glucose指数，TyGindex）与葡萄糖钳夹技术对IR的比较，结果显示TyGindex诊断IR有高敏感性和高特异性[3]。本研究初步探讨HTWP患者TyG index对预测或判断其合并糖代谢紊乱的价值。 　　1 资料与方法 　　1.1 一般资料 　　2011年
