18家社区卫生服务中心临终关怀病房患者住院费用调查和分析 　　摘 要 目的：通过对上海市2013年18家临终关怀试点机构的调查分析，深入了解上海市临终关怀服务现状，为有关部门制定相关政策提供依据。方法：回顾性分析2013年18家试点机构临终关怀病房1301例出院患者在住院治疗时的医疗费用情况。结果：患者住院医疗费用总体较低，患者人均住院费用为5500.48元，日均住院费用为189.67元，医疗费用构成较为合理，差异主要与病程长短有关。结论：临终关怀服务项目的开展是符合社会需要的，其内容需要进一步完善，服务标准需要进一步规范。 　　关键词 肿瘤 临终关怀 住院费用 　　中图分类号：R48 文献标识码：A 文章编号：1006-1533（2016）02-0017-04 　　Survey and analysis of the medical expense for the hospitalization of the patients in the hospice wards of 18 community health service centers 　　ZHI Yanhong， YIN Xiangfeng 　　（Changzheng Community Health Service Center of Putuo District， Shanghai 200333， China） 　　ABSTRACT Objective： Through the investigation and analysis of 18 pilot institutions of the hospice care in 2013 in Shanghai to deeply understand the status quo of the hospice care in Shanghai to provide a basis for the relevant departments to formulate the relevant policies. Methods： The medical expenses for hospitalization of 1 301 patients discharged from 18 pilot institutions in 2013 were retrospectively analyzed. Results： The medical expenses for the hospitalization were generally lower（5 500.48 yuan per patient）， the average daily hospitalization expense was 189.67 yuan， the medical expense constitute was more reasonable and the differences of the medical expenses were related to the length of the disease. Conclusion： The development of the hospice care service items is in line with the needs of the society， its content needs to be further improved and the service criterion to be further standardized. 　　KEY WORDS tumor； hospice care； inpatient expense 　　临终关怀即为现代医学无法治愈的患者缓解痛苦、维护其尊严以及增强临终患者生理、心理状态的积极适应能力，帮助其安宁地走完生命的最后历程，并为临终患者家属提供生理、心理关怀的立体化社会卫生服务[1]。近年来，癌症一直为我国城市和居民的首要死因，我国每年已有超过140万人死于癌症[2]。而每位晚期患者，包括晚期癌症患者、晚期HIV感染者及艾滋病（AIDS）患者，他们都需要临终关怀服务[3]。 　　2012年上海市政府将舒缓疗护（临终关怀）作为政府实事项目，选取18家社区卫生服务中心进行试点工作。2013年是试点机构完整运作的第1年。本文通过对上海市2013年18家中心临终关怀病房共1 301例恶性肿瘤患者的住院费用进行调查分析，深入了解上海市临终关怀服务现状，为临终关怀政策的制定和完善提供参考。 　　 1 对象与方法 　　1.1 研究对象 　　将上海市2013年18家临终关怀试点机构的1 301例出院临终患者作为研究对象
