传染病信息2009 年第22 卷第2 期 ·73 · 肝细胞癌非手术治疗的现状和进展 杨永平 解放军第三○二医院 北京 100039 摘 要 在过去的20 多年中，肝细胞癌（hepatocellular carcinoma ，HCC）的诊断和治疗有了显著提高，但仍具有早期诊断 难、发展快、恶性程度高、预后差的临床特点。手术切除一直被认为是可能治愈HCC 的惟一方法。但70%~80%的患者确诊HCC 时已为中晚期，无手术切除时机。对不能手术切除的HCC 患者，近年发展的非手术治疗技术可延缓甚至控制肿瘤生长，延长生存 期，提高生活质量。本文对HCC 非手术治疗现状和进展进行综述。 关键词 肝细胞癌；经皮无水酒精注射；射频消融术；经皮氩氦冷冻消融术；微波凝固治疗；经导管动脉化疗栓塞；分子靶向 治疗；基因治疗；免疫治疗 中图分类号： 文献标识码： 文章编号： R735.7 A 1007-8134(2009)02-0073-05 Current status and progress in non-resection therapies for hepatocellular carcinoma Yang Yongping 302 Hospital of PLA ，Beijing 100039 ，China Abstract In the past two decades, there have been great improvements in the diagnosis and treatment of hepatocellular carcino- ma (HCC), but the problems, including difficult diagnosis at early stage, quick progression, high malignancy and poor prognosis remain to be unsolved. Surgical resection is the mainstay of treatment for HCC. But 70%~80% of HCC patients are diagnosed in an advanced stage when most potentially curative therapies, such as surgical resection, are not eligible to be conducted. In recent years, non-resec- tion therapies for unresectable HCC have been developed, which can control tumor growth, prolong survival time and improve quality of life. This review covers the current status and progress of non-resection therapies for HCC.
