某三甲医院短暂脑缺血发作临床路径的应用效果评价 　　[摘要] 目的 评价临床路径在短暂性脑缺血发作临床治疗中的应用效果及价值。 方法 选择2008年1月～2016年1月我院神经内科收治的200 例TIA 患者作为研究对象，分为对照组（路径实施前）和临床路径组（路径实施后），各100 例。对照组采用传统治疗模式，临床路径组采用临床路径治疗模式。对两组患者的满意度、医疗工作质量（临床预后）、医疗工作效率（平均住院天数）、经济效果（次均总费用、次均药费所占比、次均检查费所占比）等方面进行对比分析。 结果 临床路径组患者满意度较对照组有所提高，脑梗死发生率降低，平均住院天数、次均总费用较对照组降低，差异均有统计学意义（P0.05）。住院费用的分配结构更合理，药费比例显著降低，检查费比例有所上升（P0.01）。 结论 我院短暂脑缺血发作临床路径管理模式的应用取得了一定的成效。该模式可提高患者满意度，提高医护人员的医疗工作质量及效率，减轻患者的经济负担。 　　[关键词] 临床路径；短暂脑缺血发作；效果评价 　　[中图分类号] R197.3；R743.3 [文献标识码] B [文章编号] 1673-9701（2017）16-0152-03 　　[Abstract] Objective To evaluate the application effect and value of clinical pathways in the clinical treatment of transient ischemic attack（TIA）. Methods 200 patients with TIA admitted in Department of Neurology from January 2008 to January 2016 were enrolled in this study. The patients were divided into control group（before the implementation of path） and clinical path group（after path implementation），with 100 cases in each group. The control group was treated with the traditional treatment model. The clinical pathways were treated with clinical pathways treatment model.The patient satisfaction， quality of medical work（clinical prognosis）， medical work efficiency （the average cost of hospitalization）， the economic effect （the average total cost per time， the average proportion of drugs per timed， the average proportion of the inspection fee per time） and so on between the two groups were comparatively analyzed. Results The satisfaction rate of patients in clinical pathways group was higher than that of the control group，and the incidence of cerebral infarction decreased in the clinical pathways group. The average hospital stay and the average of total cost per time in the clinical pathways group were lower than the those of the control group， and the differences were statistically significant（P0.05）.
