新中医 2017 年2 月第49 卷第2 期 ·78 · JOURNAL OF NEW CHINESE MEDICINE February 2017 Vol.49 No.2 小切口与手法整复穿针内固定对SandersⅣ型跟骨骨折临床观察 郑银驹，张杰，范回生 深圳市龙岗区第五人民医院，广东深圳 518111 ［摘要］目的：观察小切口与手法整复穿针内固定治疗Sanders Ⅳ型跟骨骨折的临床效果。方法：将Sanders Ⅳ型跟骨骨折患 者80 例按随机数字表法分为2 组各40 例，手法组采用小切口与手法整复穿针内固定治疗，对照组采用外侧扩大 “L”切口AO 钢 板内固定治疗，对比2 组的一般手术指标、术后功能恢复情况。结果：手法组的手术时间、手术出血量、住院时间、住院费用均 低于对照组，差异有统计学意义（P 0.05）。术后3 月、6 月，手法组的Bohlers 角、Gissane 角、跟骨中部宽度丢失值与对照组比 较，差异无统计学意义（P 0.05）。术后6 月，手法组的足踝关节功能优良率87.50%与对照组的92.50% ，差异无统计学意义（P 0.05）。术后，手法组共计有关节炎2 例，对照组共计有切口感染2 例、关节炎2 例、腓肠肌损伤3 例、腓骨肌腱炎2 例，手法组 的并发症率5.00%低于对照组的22.50% ，差异有统计学意义（P 0.05）。结论：小切口与手法整复穿针内固定治疗Sanders Ⅳ骨 折效果可靠，具有并发症少、创伤小的优点。 ［关键词］跟骨骨折；Sanders Ⅳ型；小切口；手法整复；内固定 ［中图分类号］R683.42［文献标志码］A ［文章编号］0256-7415（2017）02-0078-04 DOI：10.13457/j.cnki.jncm.2017.02.025 Clinical Observation of Small Incision Combined with Manipulative Reduction and Pinning Internal Fixation for Sanders IV Type Calcaneal Fracture ZHENG Yinju ，ZHANG Jie ，FAN Huisheng Abstract：Objective： internal fixation for Sanders IV： type calcaneal fracture.
