维持性血液透析依从性的影响因素病例对照的研究 　　【摘要】 目的：探讨影响维持性血液透析依从性的相关因素及对策。方法：将接受维持性血液透析的102例患者纳入研究，调查不同依从性患者的受教育程度、年龄、婚姻状态、透析费用、性别、医患关系、心理状态、透析总时间、健康知识、社会支持情况、家庭收入等情况。 结果：小学以下、年龄60岁、未婚离婚等、自费、医患关系一般、心理状态不良、透析时间5年、健康知识缺少、无社会支持、家庭收入低在依从性差组中所占比例高于依从性好组（P0.05）。Logistic回归分析结果显示，影响维持性血液透析患者治疗依从性差的主要因素为家庭收入低（OR=4.31）、医患关系一般（OR=3.88）、健康知识缺少（OR=3.21）、不良心理反应（OR=2.91）。结论：维持性血液透析患者依从性较差，影响因素复杂，在临床实践中需要在明确血液透析治疗依从性影响因素的基础上制定针对性的干预措施，以保证血液透析规律有序地进行。 　　【关键词】 维持性血液透析； 依从性； 影响因素 　　doi：10.14033/j.cnki.cfmr.2018.10.059 文献标识码 B 文章编号 1674-6805（2018）10-0121-03 　　【Abstract】 Objective：To explore the related factors and countermeasures of compliance with maintenance hemodialysis.Method：A total of 102 patients received maintenance hemodialysis were included in the study.The educational level，age，marital status，dialysis cost，gender，doctor-patient relationship，psychological status，total dialysis time，health knowledge，social support and family income of different compliance patients were investigated.Result：Below the primary school age，more than 60 years old，unmarried and divorced，at their own expense，the doctor-patient relationship in general，bad psychological status，dialysis time for 5 years，lack of health knowledge，no social support and low family income in the poor compliance group accounted for a higher proportion than the good compliance group（P0.05）.Logistic regression analysis showed that the main factors influencing the maintenance hemodialysis patients treatment compliance were low family income（OR=4.31），the doctor-patient relationship in general（OR=3.88），the lack of health knowledge（OR=3.21），bad psychological status（OR=2.91）.Conclusion：The compliance of maintenance hemodialysis patients is poor，and the influencing factors are complex.In clinical practice，we need to establish targeted interventions based on the factors that influence the compliance of hemodialysis treatment，so as to ensure the regularity of hemodialysis. 　　【Key words】 Hemodialysis maintenance； Compliance； Influencing factors 　　Fi
