门诊2型糖尿病患者口服降糖药的应用分析 　　[摘要] 目的 分析门诊2型糖尿病患者口服降糖药的应用情况，为2型糖尿病患者的合理用药提供参考。 方法 对确诊为2型糖尿病的门诊处方进行分析，包括患者性别、年龄等；对我院2015年1～12月门诊口服降糖药的销售金额、用药频度（DDDs）、日均费用（DDC）等进行统计分析。 结果 2015年到我院就诊的 2 型糖尿病患者男性多于女性，其中≥60 岁的患者占54.23%；我院门诊口服降糖药按销售金额排序列前3位的是：阿卡波糖、格列齐特、格列美脲；按用药频度排序列前3位的是：二甲双胍、格列美脲、格列齐特；按日均费用排序前3位的是：沙格列汀、米格列奈钙、阿卡波糖。 结论 我院门诊口服降糖药主要以二甲双胍、磺酰脲类药物为主，使用基本合理，符合指南推荐。 　　[关键词] 2型糖尿病；口服降糖药；销售金额；用药频度；日均费用 　　[中图分类号] R587.1 [文献标识码] B [文章编号] 1673-9701（2016）23-0088-03 　　[Abstract] Objective To explore the application of oral antidiabetic drugs （OAD） in outpatients with type 2 diabetes， and to provide reference for reasonable medication of type 2 diabetic patients. Methods The outpatient prescriptions for confirmed type 2 diabetic patients were analyzed， including genders and ages. The sales amount， DDDs， and DDC of OAD in our hospital from January to December 2015 were statistically analyzed. Results There were more males than females with type 2 diabetes visiting our hospital in 2015. 54.23% of the type 2 diabetic patients were ≥60 y. The top 3 OAD as to the sales amount were acarbose， gliclazide， and glimepiride； as to the DDDs were DMBG， glimepiride， and gliclazide； as to the DDC were saxagliptin， mitiglinide calcium， and acarbose. Conclusion The outpatient OAD in our hospital is generally reasonable， with DMBG and sulfonylurea as the main drugs， conforming to the recommendation in guidance. 　　[Key words] Type 2 diabetes； Oral antidiabetic drugs； Sales amount； Daily dose； Daily cost 　　随着人们生活水平的提高、饮食及生活节奏的改变，糖尿病的发病率持续上升，已成为全世界发病率和死亡率最高的5种疾病之一。糖尿病可分为1型（即胰岛素依赖型）和2型（非胰岛素依赖型），其中2 型糖尿病至少占患者总数的90%以上，此类患者体内的胰岛功能并没有完全丧失，口服药物是其治疗的首选。本研究对我院2015年1～12月门诊2型糖尿病患者口服降糖药的应用情况进行回顾性分析，以期为临床合理用药提供用药参考。 　　1 资料与方法 　　1.1 一般资料 　　采用我院计算机网络管理系统进行检索，选择我院2015年1～12月门诊确诊为2型糖尿病患者的相关数据，包括性别、年龄、门诊口服降糖药的药品名称、规格、销售金额、消耗总量等数据。 　　1.2 方法 　　应用Excel软件对我院门诊2型糖尿病患者性别、年龄、口服降糖药的品种、消耗金额、消耗数量、限定日剂量（DDD）、用药频度（DDDs）、日均费用（DDC）进行分类、统计排序。DDD值以《中华人民共和国药典》（2010年版）、《新编药物学》（17 版）[1]等权威书籍和药品说明书推荐的成人常规剂量并结合临床习惯用量。DDDs=该药品的年消耗总量/该药的DDD值，其
