临床与病理杂志 2018, 38(5) 972 J Clin Path ol R e s doi: 10.3978/j.issn.2095-6959.2018.05.011 View this article at: /10.3978/j.issn.2095-6959.2018.05.011 嗜酸粒细胞性胃肠炎患儿胃黏膜肥大细胞数量和 IL-5表达的变化及其临床意义 钱淑怡，杨辉，朱航，吴润秋，金玉，刘志峰，练敏 (南京医科大学附属儿童医院小儿消化科，南京 210008) [摘　要] 目的：探讨嗜酸粒细胞性胃肠炎 (eosinophilic gastroenteritis ，EG) 患儿胃黏膜肥大细胞 (mast cell ， MC) 数量和IL -5 表达的变化及其临床意义。 方法：选取2015 年7 月至2016 年 8月在南京医科大学附 属儿童医院消化科住院，参照 Tal ley 标准诊断为EG 的20 例患儿为研究对象，设为EG 组，其中男 16例，女4 例，年龄0.44~13.58(6.38 ±4.59) 岁。选取同期外周血嗜酸性粒细胞 (eosinophil ，EOS) 计 数及总 Ig E 测定正常且胃镜下多点取胃窦活组织行嗜酸细胞计数，EOS10 个 /HPF 的儿童为对照 组，共20 例，男 15例，女5例，年龄 1.08~12.00(5.46±3.27)岁。两组行外周血EOS 计数及总免疫球 蛋白E(IgE) 水平测定，胃镜下留取胃窦黏膜组织进行HE 染色、甲苯胺蓝染色，观察胃黏膜EOS 数 量、MC 数量变化，荧光定量PCR检测胃黏膜IL -5 mRNA表达。 结果：EG 患儿外周血EOS计数及总 IgE水平较对照组显著增高 (P 0.05) ；胃黏膜组织中EOS及MC 数量较对照组显著增加 (P 0.05) ；胃 黏膜组织IL -5 mRNA 的表达比对照组显著增高(P 0.05) 。结论：胃黏膜MC 数量增加可能导致其释 放IL -5增多，促进EOS 在胃黏膜聚集、活化，释放毒性因子造成黏膜组织损伤。 [关键词] 嗜酸粒细胞性胃肠炎；肥大细胞；白细胞介素5 ；嗜酸性粒细胞；儿童 Changes of the number of mast cell and IL-5 expression in gastric mucosa of children with eosinophilic gastroenteritis and their clinical significance QIAN Shuyi, YANG Hui, ZHU Hang, WU Runqiu, JIN Yu, LIU Zhifeng, LIAN Min (Department of Pediatric Gastroenterology, Children’s Hospital Affiliated to Nanjing Medical University, Nanjing 210008, China) Abstract Objective: To investigate the changes of the number of mast cell and IL-5 expression in gastric mucosa of children with eosinophilic gastroenteritis (EG) and their clinical significance.
