People not only pay a lot for the printing expenses, but also spend great deal of time and energy on giving out，recycling and counting the surveys in the traditional model. In addition，the controllability of questionnaires is poor and it tends to result a series of problems，such as omission and cancellation surveys. With the increasing development of the Internet and people’s living standards，the online survey has applied to a lot of aspects in many fields. To collect information through online questionnaires has become an indispensable method of investigations. It is essential for setting up a safe, convenient and high efficient online survey system at the present time when the network is developing rapid. This system is based on the basis of information technology，aimed at constructing a system which contains online questionnaire designing, survey handing out，investigation, recycling, and counting. It is used to solve a few disadvantages of the traditional questionnaire by improving the efficiency and safety of the investigations.
