泰山医学院硕士学位论文 泰山医学院硕士学位论文 万方数据 万方数据 复杂胫骨平台骨折的手术疗效评估 研究生：胡 波 专 业：外科学（骨科） 导 师：张 辉 教授 中文摘要 目的 对复杂胫骨平台骨折不同手术方法进行疗效评价 方法 从 2010 年 1 月至 2013 年 6 月分别对 21 例复杂胫骨平台骨折进行双钢板或者三 钢板内固定，其中男 13 例，女 8 例，根据 Schatzker 分型，V 型 14 例，VI 型 7 例； 联合采用三柱分型，骨折累及内外侧柱者 11 例，累及 3 柱者 10 例。三钢板固定 6 例， 双钢板固定 15 例。术后根据 HSS 评分、Rasmussen 评分以及胫骨内翻角和后倾角测 量等指标，对骨折治疗的临床效果进行评估。 结果 本组 21 例随访时间 12-21 个月，平均 15.8 个月。最后骨折愈合时间 12-17 周， 平均 14.2 周。术后即刻，3 个月，12 个月测量胫骨平台内翻角及后倾角，差异无统 计学意义。末次随访时，膝关节功能 HSS 评分为 80-95 分，平均为 86.5 分，膝关节 总屈伸度 110-145 度，平均 128.5 度。术后 Rasmussen 评分为 13-18 分，平均 14.9 分， 优 5 例，良 16 例。 结论 就复杂胫骨平台骨折的治疗方式、方法而言，通过准确的术前评估，术中恰当的 处理，选择合适的手术入路及恰当的内固定方法，在骨折的预后，将会得到及其满意 的骨折愈合及膝关节功能恢复效果。 关键词：复杂胫骨平台骨折 三柱理论 HSS 评分 Schatzker 分型 1 THE EFFECT ASSESS OF OPERATION FOR COMPLEX FRACTURE OF TIBIAL PLATEAU Postgraduate: HU Bo Profession: Orthopedics Supervisor : Prof. ZHANG Hui Objective  ABSTRACT To evaluate the curative effect of different operation methods for complex fracture of tibial plateau. Method 21 cases of complex fracture of tibial plateau were treated with two or three plate internal fixation from 2010 January to 2013 June（, male 13,female 8） including 14 of type V and 9 of type VI by Schatzker classification. According to the three column theory, 11 cases of double column fracture and 10 cases of three column fracture were included. 6 cases were treated by three plates and 15 cases were treated by two plates. During the treatment and follow-up period, the curative effect was evaluated by using Rasmussens radiological grading and HSS grading and the tibial plateau angle （TPA） or posterior slope angle （PA）. Result All patients obtained follow-up and the average follow-up time was 15.8 months （range, 12-21 months）. The average fracture healing time was 14.2 weeks （range,12-17 weeks）. No statistical difference was found in either the tibial plateau angle （TPA） or posterior slope angle （PA） when comparing the measuring results at instant, 3rd month and 12th month on radiographies.
