快通道胸腔镜简单心脏手术的临床观察 张建卿 王国锋 陈现杰 刘洋 魏金聚 郑丽娜 胡亚兰 郑州市第七人民医院 郑州市心血管病医院 卫计委介入与心血管外科内镜与微创医学培训基地（河南郑州） 心外科 450016 摘要 目的 观察快通道胸腔镜心脏手术技术在简单心脏病治疗中的疗效。方法 对2012年6月-2013年6月我院简单心脏病患者行快通道胸腔镜心脏手术，术中行快通道麻醉拔管，术后行综合管理。结果 快通道胸腔镜心脏手术手术时间3.5-6.5小时（平均4.5小时），主动脉阻断时间25-72min（平均34min）,出血量30-260ml（平均63ml），意识清醒时间10-360min（平均25min），肌力恢复情况30min后3级以上肌力占90%，呼吸机辅助时间12-40min（平均29min）、气管插管拔除时间术后30-120min（平均75min）、住院时间4-12天（平均6.5天）、住院费用总计2.5-4.5万（平均2.8万）。结论 胸腔镜心脏手术、快通道麻醉、体外循环、术后管理各环节之间应达到无缝连接，保证快通道技术顺利完成。 关键词：胸腔镜 心脏手术 快通道。 Clinical observation of fast channel thoracoscopic simple cardiac surgery Zhang Jian-qing Wang Guo-feng Chen Xian-jie Liu Yang Wei Jin-ju Zheng Li-na Hu Ya-lan Seventh Peoples Hospital of Zhengzhou,City Cardiovascular Hospital of Zhengzhou City,National Health and Family Planning Commission to intervene and cardiovascular surgery and minimally invasive endoscopic medical training base(Zhengzhou) Cardiac Surgery 450016 Abstract Objective To observe the effect of fast channel thoracoscopic cardiac surgery technology in a simple treatment of heart disease. Methods Simple heart attack patients in our hospital treat with fast channel thoracoscopic cardiac surgery from June 2012 to June 2013, with fast channel intraoperative anesthesia extubation,and with Integrated management of postoperative.Results The operation time 3.5-6.5 hours (mean 4.5 hours), aortic clamping time 25-72min (average 34min), blood loss 30-260ml (average 63ml), conscious time 10-360min (average 25min), 30min muscle recovery after three or more accounted for 90 percent strength, ventilation time 12-40min (average 29min),intubation time after removal 30-120min (average 75min), length of stay 4-12 days (average 6.5 days), total hospital costs 2.5-4.5 Wan (average 28,000). Conclusion Thoracoscopic cardiac surgery, should achieve a seamless connection between the fast-track anesthesia, cardiopulmonary bypass, postoperative management, ensure the successful completion of fast-track technology.
