三、瘤样病变 概念：临床上表现为肿块，而病理学上难以确定为增生还 是肿瘤的一类病变，例如炎性假瘤， 甲状腺腺瘤 样结节，巨淋巴结增生（Castleman’s病）等。 研究进展：随着有关病例的积累及随访资料，上述病变中 有一部病变的性质已逐渐得以确定： （一）眼眶的“淋巴细胞性炎性假瘤”（假性淋巴瘤）目前已明确其本质是 淋巴瘤，属于低度恶性的粘膜相关淋巴瘤。 （二）Castleman’s病 虽不是肿瘤，但其中某些全身多发性病变最终可 能转化为B细胞性非霍奇金淋巴瘤，对此类有症状的进展性病例， 可进行适当的化疗。 （三）肺、乳腺及软组织的炎性纤维疤痕样增生性病变，目前已清楚大多 为炎性肌纤维母细胞瘤，且具有复发倾向。 （四）浆细胞肉芽肿 通常发生于上呼吸道，病灶由单一增生的成熟浆细 胞构成，目前已确定为浆细胞瘤，部分病例可同时伴有或以后伴有 浆细胞性骨髓瘤，需局部切除加放疗，并随访。 小 结 1、肿瘤的形成是一个漫长的演变过程，肿瘤早期病变的发现对我们在诊断及治疗方面提出了新的课题，需要不断摸索，总结探讨最佳的个体化的治疗方案。 2、切忌简单地将肿瘤分为良、恶性两大类，临床医师对本学科所涉及的各类肿瘤的生物学行为应分别认识，区别对待，以确定最佳的治疗方案。 * Fig. 16.35 Usual epithelial hyperplasia.
