门奇静脉断流改良术在门静脉高压症上消 化道出血治疗中应用 【摘要】目的：研究在门静脉高压症上消化道出血 疾病治疗中采用贲门周围血管离断加胃底环行缝扎手术的 治疗效果。方法：选取在笔者所在医院肝胆外科2009-2011 年收治的患有肝硬化疾病患者中60例门静脉高压症合并上 消化道出血的患者，随机分为对照组和观察组，对照组实行 传统的门奇静脉断流术，观察组实行改良门奇静脉断流术。 对两组患者住院时间、手术后并发症以及手术前后肝功能指 标的改变对比分析。结果：观察组住院时间明显少于对照组; 观察组术后再出血率少于对照组；两组术后并发症比较差异 无统计学意义；两组手术前后肝功能指标均无明显变化。结 论：在门静脉高压症合并上消化道出血治疗中实行门奇静脉 断流改良术会取得更好的治疗效果，在临床治疗具有重要的 意义，值得推行应用。 【关键词】门奇静脉断流改进术；门静脉高压；上 消化道出血；再出血 中图分类号R573. 2文献标识码B文章编号 1674-6805 (2014) 4-0127-02 Improved Door Azygos Vein Block Technique Application in the Treatment of Upper Gastrointestinal Bleeding in Portai Hypertension/SONG Chao-yang.//Chinese and Foreign Medical Research , 2014, 12 (4)： 127-128 【Abstract 】 Objective : To study the portal hypertension hemorrhage of upper digestive tract diseases treated with pericardial devascularization plus gas trie fundus suture opera tion t rea trnent. Met hod： 60 cases of portal hypertension with gastrointestinal bleeding in our hospital from 2009 to 2011 in department of hepatobiliary surgery which suffering from cirrhosis of the liver disease , 60 patients were randomly divided into control group and observation group, 30 patients used the traditional portal azygous disconnection , the observation group used with improvement of portal azygous disconnection, the length of stay, operation complications and changes of liver function before and after operation were compared and analyzed. Result： The length of stay in the observation group were significantly less than that of the control group. The incidence of bleeding in the observation group were significantly less than that of the control group .The operation complication was no signifiedntly different between two groups of patients.The liver function had on change before and after operation treatment in two groups .Conclusion： Improved door azygos vein block can get bet ter effec t in the t rea tment of portai hypertension complicated with gastrointestinal bleeding , and it is important significance in clinical treatmerit.
