一、治疗原则 正确辨认局部皮肤软组织损伤 充分清创 稳定骨折端 有效闭合伤口 消灭创面 合理使用抗生素 及时正确处理伤口，防止感染，力争将开放骨折转化为闭合骨折 二、开放性骨折分型 Gustilo-Anderson 分型, 最常用,共分三型 (表65-1) Type Ⅰ：Puncture wound ≤1cm in diameter and is relatively clean Type Ⅱ：Laceration longer than 1cm without extensive soft-tissue damage, flaps, or avulsion, with a minimal to moderate crushing component Type Ⅲ：Extensive damage to the soft tissues, including muscle, skin, and neurovascular structures 四、治疗步骤 清创越早感染机会越少，力争在伤后6—8小时内完 成清创术
