原发性肝癌治疗现状与进展 河北医科大学第二医院　白文元 liver cancer in world 流行病学 ? 美国的流行病学资料提示HCC的发病率在上升 ? 在英国，每年HCC引起约1500例死亡。 40%的病例HCC是基础肝病的首发表现 ? 在日本则80%HCC病例，在无症状期通过对已知有肝硬化患者的筛查发现 ? 全世界的HCC流行率与病毒性肝炎流行相平行，大多数病例伴有HBV或HCV感染,发达国家HCC发病率增加可能是HCV感染流行的直接结果 ? 酒精性、遗传性血色素病性和罕见的原发性胆汁性肝硬化亦有相关性 流行病学 原发性肝癌是严重威胁人类健康的恶性肿瘤之一 WHO统计，全球每年新发肝癌病例100~150万例 全世界45％以上的肝癌发生在我国。 国家卫生部1999年统计 自90年代以来原发性肝癌已上升为恶性肿瘤的第二位, 在城市仅次于肺癌,在农村仅次于胃癌 全球每年约有31万患者, 中国每年有13万患者死于肝癌，占全球肝癌死亡总数的42% 深入研究肝癌的预防与治疗，降低其死亡率是我们的使命 肝癌治疗历史回顾 50年代肝癌临床的进展(大肝癌切除) 主要是建立在解剖和生化进步的基础上; 60-70年代肝癌临床的进展（肝移植和小肝癌） 主要建立在免疫学进步的基础上； 80年代肝癌临床的进展（二期切除、局部治疗等） 则以电脑、新技术为主要研究背景； 90年代生物学治疗的进步和推动。 国外肝癌治疗现状 The optimal management of hepatocellular carcinoma depends on a variety of factors including the size, number, and distribution (unilobar vs. bilobar ) of tumors, the relationship of The tumor to hepatic vasculature(肝癌血管) the status of distant metastases (远处组织关系) the severity of liver disease (Child-Pugh score), the suitability of the patient for liver transplantation, (移植适应症?) the functional status of the patient, and local expertise. (病人肝功 专家意见) The mean survival of symptomatic patients with hepatocellular carcinoma is approximately 2–3 months(大约 病程) The doubling time of the tumor size is 2–3 months. Optimal management should attempt to prolong without compromising quality of life. (最佳 生命质量) Hepatocellular carcinoma is relatively insensitive to systemic chemotherapy or radiotherapy and, therefore, these options will not be discussed.
