大 埔 三 育 中 學 Tai Po Sam 中一自行分配學位 二零零九至二零一零年度入學申請 Application for Admission to 2009 – 2010 S.1 Discretionary Place A. 報名須知 Notes to Applicants 1. 大埔三育中學是一所直接資助英文中學，2009 –2010年度中一每月學費為港幣350元(全年10期)。 Tai Po Sam Yuk Secondary School is a Direct Subsidy EMI school. The school fee for Secondary 1 in 2009 – 2010 will be HK$350 per month. (10 installments a year). 2. 申請表格須於指定日期內交回本校校務處，日期為2009年1月 (申請表格於2008年11 Application form should be submitted to our school office within the application period: 2nd January 2009 to 19th January 2009.( Application Forms are available on 9th November, 2008 ) 3. 面試日期︰2009年1月 Date of interview: 18th January, 2009 (Sunday) 4. 本校會將申請名單呈交教育局查核。 Your application form will be transferred to EDB for verification. 5. 本校在2009-2010年度自行分配學額數目為40，獲本校取錄的應屆小六學生，教育局將在中央派位時（二零零九年七月七日）另行通知。非應屆小六學生或擁有居港權的國內學生將會在2009年3月前獲得通知。 The forty successful applicants, who are primary 6 students, for the 2009 S.1 discretionary places will be notified by EDB on 7th July 2009 during the central allocation. Those who are not primary 6 students or from mainland China with Hong Kong ID card will be notified by our school before March 2009 . 6. 申請人必須呈交最近兩年的成績表影印本。如有獎狀或其他證書，請交影印本。 All applicants must submit copies of their report cards of the latest two years. Copies of awards or certificates may also be submitted. 7. 所有申請表格及證明文件副本將不退回，校方絕對保密。所有資料將會在2009 銷毀。 All materials submitted to school for application will not be returned, but will be treated confidentially. The materials will be destroyed on 31st August, 2009. 8. 報名費為港幣二十元及恕不退回。 Application fee is HK$20 and will not be refunded.
