USER GUIDE 核保指引使用指南 1 - An impairment can be found either by alphabetical order or by human system. 病患資料可根據疾病名稱的首個英文字母或按人體功能系統查閱。 2 - You can look up the usual medical impairment requirement of each illness which is arranged in alphabetical order under the respective human system section. 閣下可從目錄 (按人體功能系統分類），按英文字母順序排列之各項病患資料，尋找有關之醫療核保規定。 3 An Attending Physician Statement (APS) ordering guideline If APS is needed after check with the following 3 APS classes, please get the followings: i) for insured = adult with age 18 or above, please submit Original consent form + patient card copy + ID copy; ii) for insured below age 18, please submit Original Consent Declaration of custody form + patient card copy + Parents ID copy, birth certificate / ID of insured. 索取應診醫生聲明書(APS)所需指引 基於以下三類之應診醫生聲明書(APS)指引，若需索取APS ，請一同遞交以下文件： i) 18歲或以上之成年受保人，請遞交客戶之授權書＋覆診卡副本＋身分證副本； ii) 18歲以下之受保人，請遞交客戶之授權書連同監護聲明書＋覆診卡副本＋父母身分證副本＋出生證明文件／受保人身分證副本。 APS CLASS 類別 A Usually do not request, unless, this health history leads to chronic health problem 不需要，除非此病歷引至慢性健康問題。 B Request if sum insured is USD500,000 per life in our Company and the last consultation/ treatment received date is within 1 year. If client can submit an updated check up report which is sufficient to prove the health condition, we can consider to waive the APS requirement. 若保障額在本公可 $500,000 美元及於過去一年曾求診，則需要； 可考慮豁免，若客人能遞交最近身體檢查報告證明健康狀況。 C Request in all situation If information obtained in APS is not sufficient for assessing the risk, will need further underwriting requirements. If client can submit an updated check up report which is sufficient to prove the health condition, we can consider to waive the APS requirement. 任何情況下均需要 若應診醫生聲明書未能提供充足資料作風險評估，將需要更進一步的核保要求； 可考慮豁免，若客人能遞交最近身體檢查報告證明健康狀況。 e.g. 例子If client suffers from pneumon
