颅脑外伤合并凝血机制异常患者的手术治疗(附4例报告)的中期报告 摘要： 背景：颅脑外伤是造成死亡和致残的主要原因之一，在颅脑外伤后合并凝血机制异常的患者中手术治疗的风险更高。 目的：评估颅脑外伤合并凝血机制异常的患者手术治疗的安全性和有效性。 方法：回顾分析2016年1月至2019年12月在我院接受手术治疗的颅脑外伤合并凝血机制异常的患者，包括基本病例资料、手术治疗、围手术期管理、病理结果等。 结果：共纳入4例患者，男性2例，女性2例，年龄在23~65岁之间。患者均有不同程度的凝血机制异常，其中2例为DIC，2例为血小板减少症。手术种类包括颅内血肿清除、颅骨创伤修复和脑室穿刺等。全组患者中，术后并发症为1例手术切口感染，未见因凝血异常导致的术后出血等严重并发症。所有患者在术后均接受严密监测和积极治疗，无1例因手术治疗死亡或发生严重并发症。 结论：颅脑外伤合并凝血机制异常的患者手术治疗是安全有效的，在围手术期严密监测和积极治疗的前提下，应积极采取手术治疗措施。 关键词：颅脑外伤；凝血机制异常；手术治疗；安全性；有效性 Abstract: Background: Traumatic brain injury is one of the leading causes of death and disability, and the risk of surgery is higher in patients with coagulopathy after traumatic brain injury. Objective: To evaluate the safety and efficacy of surgical treatment in patients with traumatic brain injury and coagulopathy. Methods: A retrospective analysis of patients with traumatic brain injury and coagulopathy who underwent surgical treatment in our hospital from January 2016 to December 2019, including basic case data, surgical treatment, perioperative management, pathological results, etc. Results: A total of 4 patients were included, including 2 males and 2 females, with ages ranging from 23 to 65 years. All patients had varying degrees of coagulopathy, including 2 cases of DIC and 2 cases of thrombocytopenia. The types of surgery included removal of intracranial hematoma, repair of skull trauma, and ventriculocentesis, etc. One case of surgical incision infection was found in the postoperative complications of the entire group, and no serious complications such as postoperative bleeding caused by coagulation abnormalities were found. All patients were closely monitored and actively treated postoperatively, and no patient died or experienced serious complications due to surgical treatment.
