在欧洲的rFVIIa辅助治疗大出血推荐指南中，除了创伤、心脏手术和产后出血外，专家还建议诺其用于传统治疗无效的外科出血。Massivebleedingisanimportantcauseofmorbidityandmortality,andeveryattemptshouldbemadetocontrolbleedingbyconventionalmeansbeforeconsideringatrialofrFVIIa.IndicationsforuseMassivebleedingisclassicallydescribedaslossof1bloodvolumein24hours.Inthecontextoftheserecommendations,weshouldconsidergreaterlossofbloodvolumes,suchaslossof50%bloodvolumeinlessthan3hours.Undertheseconditions,administrationofbloodproductswouldprecedetheadministrationofrFVIIa.ConclusionInresponsetoaclinicalneedforpracticalguidanceontheuseofrFVIIainthemanagementofuncontrolledmassivehemorrhage,consensusguidelineshavebeendevelopedbyanexpertpanelonthebasisofasystematicreviewofthecurrentevidencebase.ThereisgradeBevidencetosupporttheuseofrFVIIa,adjunctivetosurgicalcontrolofbleeding,tomanagebleedingduetoblunttrauma,gradeEevidenceofapossibleroleforthistherapyinthecontrolofpost-partumhemorrhageandgradesEandDevidence,respectively,fortheuseofrFVIIainthemanagementofuncontrolledbleedingassociatedwithsurgeryandcardiacsurgery.Atpresent,apaucityofdatafromrandomizedcontrolledtrialswithrFVIIalimitsboththestrengthandthescopeofclinicalrecommendations.GeneralrecommendationsRecommendation1.Everyattemptshouldbemadetocontrolbleedingbyconventionalmeans.rFVIIashouldnotreplaceand/ordelaysurgeryoranyothermethodsusedtocontrolthesourceofbleeding,suchasangiographywithembolization.GradeERationale1.rFVIIawillbeeffectiveonlyoncesourcesofmajorbleeding(suchasopenbloodvessels)havebeenclosed.Oncemajorbleedingfromdamagedvesselshasbeenstopped,itmaybehelpfultoinducecoagulationinareasofdiffusebleedingandoozing.Recommendation2.Traditionaluseofbloodproducts,includingRBCs,platelets,FFP
