PREAMBLE

ThiscontractismadebyandbetweenPartyAandPartyB,whodesirestoreceivemedicaltreatmentfromtheMedicalInstitution.Thepartiesherebyagreeasfollows:

ARTICLE1:SCOPEOFSERVICES

1.1PartyAshallprovidemedicalservicestoPartyB,whichshallinclude,butnotbelimitedto,thefollowing:

-ExaminationanddiagnosisofPartyBsmedicalcondition.

-Prescriptionofappropriatetreatmentandmedications.

-Follow-upcareandmonitoringofPartyBsprogress.

-AdviceandconsultationonPartyBshealthandwell-being.

1.2PartyAshallalsoprovideanyothermedicalservicesthatmaybereasonablyrequestedbyPartyB,subjecttotheavailabilityofresourcesandthenatureoftherequest.

ARTICLE2:DurationofServices

2.1TheservicesprovidedbyPartyAshallbeforaperiodof[DurationofServices],commencingontheDateofContractandendingontheexpirationdatespecifiedinthiscontract.

2.2Ifeitherpartyrequiresanextensionoftheserviceperiod,theyshallmutuallyagreetosuchanextensioninwriting.

ARTICLE3:FeesandPayment

3.1ThefeesforthemedicalservicesprovidedbyPartyAshallbe[MedicalFeesperSession].ThisfeeshallbepayableinadvanceforeachsessionbookedbyPartyB.

3.2PartyBshallalsoberesponsibleforanyadditionalexpensesincurredduringthecourseoftreatment,suchasprescribedmedications,medicalsupplies,andanyotherreasonableexpensesrelatedtothetreatment.

3.3AllfeesandexpensesshallbepaidinfullbyPartyB,eitherdirectlytoPartyAortoadesignatedpaymentaccountasagreeduponbytheparties.
