[Abstract]ObjectiveToinvestigatethecerebroprotectiveeffectofpre-filledsivelestatsodiumduringcardiopulmonarybypass（CPB）inpatientswithvalvereplacementsurgery.MethodsFromJanuary2022toDecember2023，104patientswereincludedandrandomlydividedintoexperimentalgroup（n=54）andcontrolgroup（n=50）.Thesivelestatsodium4.8mg/kgwasmixedtothepre-filledsolutionsforCPBinexperimentalgroup，whileanequalamountofphysiologicalsalinesolutionwasmixedincontrolgroup.Internaljugularvenousbloodwascollected5minutesafteranesthesiainductionand10minutes，4hours，24hours，and72hoursaftertheaorticcross-clampwasreleasedtodetectplasmaS-100proteinandinterleukin（IL）-6concentrations.Dataonoperationtime，CPBtime，meanarterialpressure，andhematocritwerealsorecorded.ResultsTherewerenostatisticaldifferencesinoperationtime，CPBtime，assistedcirculationtime，meanarterialpressure，andhematocritbetweentwogroups（Pgt;0.05）.Afteranesthesia，therewerenosignificantdifferencesinS-100proteinandIL-6betweentwogroups（Pgt;0.05）.After，theplasmaconcentrationsofS-100proteinandIL-6increasedinbothgroups，buttheconcentrationsofS-100proteinandIL-6inexperimentalgroupweresignificantlylowerthanthoseincontrolgroupatearlystages（10minutes，4hours，24hours）aftertheaorticcross-clampwasreleased（Plt;0.05）.ConclusionSivelestatsodium，asacerebroprotectivedruginCPBheartsurgery，canreducethelevelsofS-100proteinandIL-6，demonstratingaprotectiveeffectoncentralnervouscells.
