[Abstract]ObjectiveToassessthevalidityandutilityofthefour-columnconceptbycomparingtheimagingresultsandclinicalefficacyofthefour-columnconceptwiththeSchatzkertypingconceptinthetreatmentoftypeI-IVtibialplateaufractures.MethodsAtotalof70tibialplateaufracturepatientsselectedfromDongguanHospital，GuangzhouUniversityofTraditionalChineseMedicinefromSeptember1，2022toApril30，2023，whoweredividedintotreatmentgroupandcontrolgroup，with35patientsineachgroup.AllpatientsunderwentcompleteX-rayandCTthree-dimensionalreconstructionexamination，andregularpostoperativereviewoforthopantomogramsofthekneewasperformed.Datawerecollectedat1monthand1yearpostoperatively，includingmeasurementsofposteriortibialslope（PTSA），tibialplateauanterior（TPA），andsecondarydisplacementofthearticularsurfaces，aswellasthevisualanaloguescale（VAS），rangeofmotion（ROM），andLysholmkneescorescale（LKSS）.ResultsTheoperationdurationandbleedingvolumeintreatmentgroupwerelessthanthatincontrolgroup，andthedifferencewerestatisticallysignificant（Plt;0.05）.Thereweresignificantdifferentesat1monthafterpostoperativelyinkneeROMandLysholmscoresbetweentwogroups（Plt;0.05）.ThereweresignificantdifferencesinPTSAandarticularsurfacecollapseat1monthand1yearpostoperatively（Plt;0.05）.ConclusionSurgicaltreatmentofSchatzkertypeI-IVtibialplateaufractureundertheguidanceofthefour-columnconceptcaneffectivelyrestorethealignmentoffractureendsandfixationstrength，andimprovethemobilityofthekneejoint，whichisworthyofpopularizationandapplicationinthecli
