首发精神障碍患者家属病耻感发展轨迹及预测因素分析

[摘要]目的：分析首发精神障碍患者家属病耻感发展轨迹的潜在类别及其影响因素，为临床护理提供参考。

方法：采用便利抽样法，选取2021年1月至2023年12月绍兴市某三级甲等精神病专科医院收治的200例首发

精神障碍患者家属作为研究对象，采用贬低-歧视感知量表评估患者出院时、随访1个月、随访3个月、随

访6个月时患者家属的病耻感水平，通过增长混合模型分析类别最优数量并命名，采用Logistic回归分析潜在类

别差异的独立影响因素。结果：200例首发精神障碍患者家属病耻感发展轨迹可分为4个类别，分别为轻度病耻

感加重型、轻度病耻感改善型、中等病耻感改善型、高度病耻感改善型。年龄、文化程度、家庭人均月收入、与

他人共同照顾、职业、社会支持评分、患者疾病类型是首发精神障碍患者家属形成不同病耻感发展轨迹的独立影

响因素（均P0.05）。结论：首发精神障碍患者家属病耻感发展轨迹存在异质性，且受年龄、文化程度、家庭人均

月收入、与他人共同照顾、职业、社会支持评分、患者疾病类型等因素的影响。
