  肝郁化火证的研究进展# 王椿野1，郭蓉娟2** （1. 北京中医药大学东方临床学院，北京 100078； 5 10 2. 北京中医药大学东方医院脑病二科，北京 100078） 摘要：肝郁化火证是中医情志致病理论重要病机之一，病因多由情志所伤，肝气郁结，致郁 而化火。肝郁化火证的病机演变比较复杂，可阳盛伤阴，还可犯胃、刑金、扰肾、冲心、甚 动风夹痰，形成更复杂的病机。肝郁化火证属于里实热证，治疗时除了清肝泻火，还要疏肝 解郁。目前，对于肝郁化火证的基础研究主要涉及炎症、神经-体液调节、基因和性格等方 面。一般认为，肝郁化火时，机体处于炎症反应状态；神经-体液代谢失调；交感神经机能 偏亢；HPT 轴激素分泌紊乱；机体代谢旺盛，能量储备减少。肝郁化火证患者的易感因素 可能与 5-HTTLPR、TPH 基因多态性关系较密切，心理学也证实了 TABP 之人肝火证的易感 性。本文就目前肝郁化火证的中医病因病机，及其相关的现代本质研究进行总结讨论。 关键词：肝郁化火证；病因病机；炎症；神经体液调节；人格 15 Research Progress of Liver Fire Caused by Emotional Problems WANG Chunye1, GUO Rongjuan2 (1. Beijing University of Chinese Medicine East Hospital Second Department of Encephalopathy,Beijing 100078) Abstract: Syndrome of stagnation of liver fire is one of the important emotional pathogenic theories of Traditional Chinese Medicine pathogenesis. It’s caused emotional problems, liver Qi stagnation, which transforms into fire. Evolution of pathogenesis of liver stagnant fire is very complex, hyperactive Yang injuring essence of Yin, but also attacking the stomach、lung、kidney and heart, even forming phlegm and wind pathogenic factor, leading more complex pathogenesis. Syndrome of stagnation of liver-fire belongs to the syndrome of excessive heat in liver, treatment in addition to clearing liver and purging fire, course the liver and resolve depression. For now, basic research of syndrome of stagnation of liver fire mainly related to inflammation, neurohumoral regulation, gene and the personality, and so on. Generally, the body in the inflammatory state; neuro-humoral disorder, hyperfunction of sympathetic nerve function; HPT axis hormone secretion disorder；massive metabolism, reduced energy reserves. Predisposing factor in patients with syndrome of stagnation of liver fire is related to 5-HTTLPR, TPH gene polymorphism and TABP.
